The LPN Case Manager will employ a process of case management to communicate, coordinate, report, and collaborate with clients, families and providers involved in the client’s care. He or she will assist with monitoring the quality of care, services delivered to the client to ensure that the goals of the plan of care are being achieved by assisting with the individual care plans developed in collaboration with the Registered Nurse Physician, Caregivers, Families, and the Care Team.
The LPN Case Manager/Field Nurse will:
- Ability to analyze, organize, and prioritize work accurately while meeting multiple deadlines.
- Provide Case Management for Medicare and Private Duty Clients.
- Conduct client field visits as needed to provide nursing care in the field.
- Assist with the planning, coordination, and delivery of interdisciplinary home health care services by ensuring compliance with federal/state standards and clinical best practices.
- Assist with reducing the need for hospitalization or rehospitalization.
- Provide orientation, interviews for HHA and LPN candidates.
- Work with the Clinical Director to ensure that Regulatory requirements daily are met.
- Work with Physicians office to ensure CHAP and Medicare Standards are met.
- Work with Therapy company to schedule and receive care notes in a timely fashion.
- Assist with developing a care plan.
- Perform other assigned duties as needed.
Qualifications and Experience Required:
- LPN License required in the state of Florida
- A minimum of 3-5 years of nursing experience
- Must be computer literate and have proficiency in Microsoft and or Google Suite
- Prior experience in working with Kinnser and WellSky preferred
