Job Title: Parenting & Community Liaison Social Worker

General Description of Duties: 

Using a strengths-based model, the Parenting & Community Liaison Social Worker coordinates Crittenton’s Aftercare Parenting Education Program (A.P.E.P.) Provide home-based services and support to former Crittenton clients and their families who need help promoting positive development, meeting challenges and preventing adverse outcomes, with a special emphasis on self-sufficiency for all family members.   Proactively works to cultivate relationships with community stakeholders and referral sources to increase referrals and strengthen Crittenton’s relationships with our community partners. 

Relationship to agency mission: 

Responsible for designing and implementing a plan for each participant to attain the maximum opportunity for successful parenting. 

Line of Authority: 

The Parenting & Community Liaison Social Worker reports directly to the Social Work Supervisor. 

Job Classification: 

Full time, nonexempt 

Beginning Hourly Pay Range:  $20 - $22 per hour

Specific Duties and Responsibilities:  

Conduct a minimum of one home visit per month to each A.P.E.P. participant to provide support so each family member can reach their educational, vocational and health goals; assess the need for increased home visits based on acuity level.

  • Develop a written service plan in partnership with the participant within 30 days of initial home visit; service plans shall address the full range of participant needs, including: 
    • Educational 
    • Vocational 
    • Housing
    • Relational 
    • Familial 
    • Medical and health education 
    • Nutrition 
    • Family planning 
    • Individual and/or family counseling 
    • Identification of services to be provided by referral to another agency. 
  • Engage current Crittenton clients to participate in the A.P.E.P. program by establishing trust in accepting services during their stay as a resident in the Maternity Program; work collaboratively with Maternity Program staff to discuss best approaches for each client. 
  • Using an individualized, strengths-based approach, work to motivate participants to make positive lifestyle choices, increase their positive self-esteem, and to have a desire to improve their situation. 
  • Seek to eliminate the occurrence of family violence, abuse, and neglect by following/developing program content to educate and coach participants on elements of supportive parenting, effective parenting skills, alternatives to corporal punishment, social skills and appropriate coping strategies. 
  • Educate participants about their child(ren)’s development using the Ages and Stages assessment tool; refer participants to specialized agencies for early intervention when developmental deficiencies are suspected. 
  • Educate participants on the importance of routine medical care, immunizations, well-baby/child visits, annual physicals and dental hygiene for all family members; inform participants of transportation assistance resources for keeping medical/dental appointments. 
  • Provide preventive health care information including, but not limited to, STD’s, HIV/AIDS, birth control, substance abuse, and reducing risk-taking behaviors; refer participants to appropriate healthcare professionals for treatment needs. 
  • Identify and help participants develop formal and informal community supports.
  • Co-facilitate parenting classes and life skills classes for current residents.
  • Develop and maintain collaborative relationships with community agencies that have resources and/or services that would benefit participants. 
  • Publicize A.P.E.P. participant needs and work to secure donations for filling those needs. 
  • Develop and maintain a consistent communication and follow-up plan to keep current and potential participants informed and engaged. 
  • Utilize the Family Strengths/Needs Assessment checklist and adapt approaches to delivering services based on individual participant’s strengths. 
  • Establish and maintain APEP records by documenting all program activity, participant contacts and visits, programming curriculum, progress, outcomes, and outputs.
  • Generate and compile statistical reports related to APEP outcomes and agency Program Logic model.  
  • Cultivate relationships with community stakeholders and referral sources; represent Crittenton in the community by attending outreach events and other functions.
  • Play an active role in the Outreach Committee.
  • Coordinate transportation as needed for current residents and APEP participants.
  • Support Volunteer/Vocational Coordinator and other departments as needed. 
  • Other duties as assigned. 

 

Qualifications: 

  • Ability to engage, establish trust, develop relationships, motivate, and collaborate effectively with individuals and families from a wide range of backgrounds, cultures, and perspectives. 
  • Possess the competencies needed to provide services in a culturally competent manner. 
  • Must be able to identify concerns related to substance use, mental health, domestic violence, and child abuse and neglect. SW must have a working knowledge of referring participants to the appropriate community services.
  • Ability to link participants with needed services offered by other community providers. 
  • Must be able to observe and understand child, individual and family functioning. 
  • Ability to work a flexible schedule that includes frequent nights and weekends. 
  • Must be able to work as part of a team. 
  • Ability to move through building to check rooms, evacuate building and intervene with clients. 
  • Must pass a thorough background check which includes criminal history, drug test and driving record. 
  • Valid Driver License required. 
  • Strong interpersonal and organizational skills with a customer focused attitude.

Education and Experience: 

  • Bachelor’s degree in social work or related field 
  • One year of experience in the field of human services or social work
