Role:
The HIM Assistant position is responsible for the accurate, timely and complete process of medical records.
• Key Responsibilities
o Answer & triage phone calls to appropriate staff
o Cross train to support various other departments as needed i.e., IT department overflow, Central Supply and Care Services intake staff on evenings and weekends.
o Complete monthly current audits on active patients for compliance
o Complete final audits on all discharged and deceased patients within 48 hours.
o Develop collaborative relationships within the organization and outside the organization for efficient flow of information.
o Ability to work in other locations as needed
o HIM II Participate on projects with HIM management
o HIM II Face to face physician scheduling
o HIM II Understand ICD-9 codes. Review physician's documentation to ensure adequate coding is utilized.
o HIM II Cover for HIM I staff and to help with training as needed.
o Support the EMR software applications
o Create and maintain training material for Suncoast users and documentation for report writing.
o Train all new staff in the utilization or Suncoast and Fortis (AllDocs)
o Must be flexible to work in other locations if needed.
• Knowledge & Skills
o Intermediate to advanced computer skills
o Excellent communication; via phone, in person and email
o Attention to detail & ability to handle confidential information following HIPPA guidelines
o Problem solving and time management

• Required qualifications
o High School Diploma or GED required
o 2 years working with medical records or medical office experience required
o II) Degree in Health Information Management, preferred or combination of education & medical office experience
o Move or lift up to 10 lbs, may include with reasonable accommodations
o Valid Florida Drivers' License with good record

o Clear level 2 background check

Certificates/Licenses/Registration
Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation.
Experience Requirements
At least 2 years prior secretarial experience, medical office and terminology experience is required. Must possess excellent customer service skills. General computer knowledge is also required. Must be able to quickly learn other computer programs
Position Specific Requirements (not all inclusive)
Answer phones utilizing best practice phone skills, showing ability to accurately problem solve and triage phone calls to appropriate staff.
Demonstrate competency with a wide variety of software programs as well as the ability to move information from program to program. Mentor clinical staff with the usage of programs.
Demonstrate time management and organizational skills, as well as the ability to prioritize multiple tasks for successful outcomes.
Understand HIPAA guidelines of privacy and security, monitor work areas for compliance in this area and report to senior management when non-compliance is detected.
Develop collaborative relationships within the organization and outside the organization for efficient flow of information. Show the ability to secure information electronically from outside facilities i.e. FCS, Internal Medicine, Lee Memorial Health Systems etc., as needed and permitted then electronically moving the information within Hope's system.
Cross train to support various other departments as needed i.e., IT department overflow, Central Supply, and Care Services intake staff on evenings and weekends.
Secure completed documentation in a timely manner for admissions. Determine legal/financial documentation is appropriate and compliant. Compare admission documentation and EMR entry for consistency of information. Complete initial audit within 24 hours of admission and document any deficiencies for staff follow-up. Notify management daily of missing/incomplete documentation.
Complete monthly current audits on active patients for compliance in meeting regulatory standards and document any deficiencies for staff follow-up. Notify management daily of missing/incomplete documentation.
Complete final audits on all discharged and deceased patients within 48 hours. Notify patient discharges and deaths daily to appropriate staff and community physicians, update EMR to reflect patient status, and ensure all correct documentation occurs. Ensure death certificates are complete and the correct information is entered, signed by physician and returned to funeral homes; troubleshoot any problems that might arise from death certificates not being completed correctly.
Make sure all Certifications, Re-Certifications and orders are signed by physicians using the correct dates and ensure signature standards are being maintained. Mail Care Plans monthly to community physicians and ensure appropriate documentation is completed.
Make changes to EMR regarding patient information, maintain/verify all EMR management and trending reports, maintain/monitor transfer to Hospice House report and the bed board. Where appropriate verify the patient task in EMR is correct. Notify appropriate staff when information is erroneous.
Ensure patient documentation is scanned into Fortis daily.
Assist clinical staff with patient DME needs by placing orders and ensuring information is complete and accurate. Ensure EMR is updated.
Setup and coordinate transportation of patients as needed. Maintain the transportation invoices.
Order and maintain supplies for teams. Maintain the supply room and inventory. Order supplies utilizing appropriate software programs for Purchasing Requisitions and Receipt of Items.
PHI for continuity of care that has been sent or received must be tracked in EMR in order to stay compliant with HIPAA. Ensure security of paper records and monitor use of electronic record.
Manage the sale of Expression of Hope items, maintaining a record and keeping track of money.
Process incoming mail and interoffice for the home and/or house teams accordingly. Deliver to the appropriate staff. Understand the postal system for outgoing correspondence.
Create Maintenance requests and follow-up to ensure requests are completed. Keep management informed of progress or process.
Match, scan and enter the 837 Encounter Data information.
Complete all necessary chart assembly, scanning indexing, etc. for Hope VNA.
Contract Management for multiple programs.
Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
Consistently represents Hope with integrity and professionalism.
Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service.
When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.
Assist with special projects, as requested.
All other job duties as may be assigned.
