Position Summary:
Under the supervision of the PACE Transportation Coordinator, the PACE Driver is responsible for the direct transportation of PACE participants to and from their destinations including, but not limited to, transportation to/from the center/day clinic and to/from appointments related to approved PACE covered services. The driver has continuous interaction with participants, caregivers/families and community agencies and plays an integral part in communicating changes in participants to the PACE Transportation Coordinator for input to the Interdisciplinary Team (IDT).
Position Duties & Responsibilities:

• Effectively communicates with participants, families, staff and other agencies related to the efficient, effective and safe transport of PACE participants.
• Drives a specially equipped van to deliver a frail, elderly and/or physically limited population to varied destinations. Assures that wheelchair lifts and tie down equipment are properly operated and that participants are delivered to appointments in a safe and timely manner.
• Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes in collaboration with the Transportation Coordinator.
• Assists participants on and off the van in a safe and courteous manner. Deals with medical equipment and other emergencies that may arise.
• Inspects and maintains van in a clean, orderly, and safe condition. Reports any needs for maintenance.
• Participates as a member of the Interdisciplinary Team (IDT) by reporting changes in participant condition, concerns or updates to the Transportation Coordinator frequently and as necessary.
• Attends staff meetings and takes part in participant care planning as needed.
• Observe each participant for any change in physical, mental, emotional, and social functioning and shall report such changes to the Transportation Coordinator and/or other IDT members as needed.
• Completes documentation as required.
• Participates in an on-call rotation.

Position Qualifications:

• High school diploma or general education degree (GED) required.
• 1 year experience transporting individuals with special mobility needs.
• Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
• Able to work positively in a diverse and stressful environment.
• Must have, or willing to obtain, first aid training.
• Must maintain CPR certification.
• Must have a valid driver’s license in accordance with policy with proof of automobile insurance required.
• Knowledge of safety and infection control requirements for a healthcare environment.
• Knowledge of automobile and medical equipment use and maintenance.
• Ability to communicate clearly and effectively.
