GENERAL SUMMARY:
The Patient Assistance and Access Specialist is responsible for the processing of referrals and tracking for the assigned service lines. The Patient Assistance and Access Specialist ensures access to medication through Patient Assistance applications and processing medication prior authorizations for the Adult Service Line. The Specialist can be trained to be a Certified Application Counselor which identifies, educates, and supports individuals and families by assisting them with the screening and enrollment process into Medicaid, Medicare and the Health Exchanges. Active in-reach and outreach efforts are conducted in order to ensure MVHC patients and community members gain access to healthcare. The Specialist ensures access to transportation to medical appointments through scheduling transportation for patients in need.
Minimum Qualifications:
  • 1-2 years of previous physician office experience preferred
  • CPR certification required
  • Electronic Health Record knowledge
  • High School Diploma or equivalent
  • Knowledge of community resources within Muskingum and adjacent counties, especially resources serving minority and low-income populations
  • May require evenings or weekends as needed for outreach events
  • Must have reliable transportation and be willing to travel locally and periodic out-of-town travel
  • Strong organizational and communication skills necessary
  • Understanding of and sensitivity to issues of concern to low-income and minority populations
WORKING CONDITIONS:
· Average working hours with possible non-traditional hours
· Carrying up to 20 lbs occasionally
· Good working conditions in an office environment
· Lifting up to 30 lbs 4-5 times a day
· Normal coordination and/or visual attention to connection with the standard flow of work
· Sitting 1-2 hours at a time (8-10 hours a day)
· Standing 1-2 hours at a time (8-10 hours a day)
· Walking approx. 30 minutes per hour throughout the day
· Withstands pressure and deals with emergency situations as needed
DUTIES and RESPONSIBILITIES:
  • Accurately track and record all patient referrals
  • Appropriately schedule all referral appointments while coordinating with the patient and the outside providers.
  • Collaboratively work with community partners to arrange transportation for patients to all medical appointments as needed
  • Collects and collates in-reach and outreach activity and impact for reporting purposes
  • Complete and maintain all required paperwork, records, and documents
  • Complete Patient Assistance applications and track follow-up requirements
  • Creates and maintains a calendar of events for in-reach and outreach efforts
  • Document all referrals into the patient’s chart in the EPIC system
  • Educate applicants of how programs function, when and where they can use the medical coverage
  • Educate applicants of requirements and structure of programs to prevent loss of coverage
  • Educates applicants on required documents for enrollment in all programs
  • Efficiently notify the patient of their appointment and provide instructions if needed
  • Explore reason for noncompliance with referrals and assist in removing any barriers, working with the Care Coordinator
  • Performs medication prior authorizations for assigned service lines
  • Proficiently arrange and prioritize emergency referrals, surgeries, and or prior authorizations
  • Provide education and presentations to MVHC providers and staff, community organizations, agencies, business, faith-based organizations and any other groups in order to reach the target population
  • Provide follow up with referring provider via Epic for patients who are noncompliant with referrals
  • Provide general program information and perform outreach and enrollment functions in MVHC, partner agencies, and community partners
  • Screens applicants and assists uninsured patients in enrolling in applicable insurance programs
  • Send needed information to outside providers if not an EPIC provider
  • Submit, track and document medication and surgical prior authorizations
  • Support other Health Center activities and perform other duties as assigned
  • Track all assigned monthly statistics
  • Track enrollment and effective dates of coverage to help applicants prevent lapse of coverage.
  • Verifies accurate insurance information
  • Work as a liaison to community organizations, agencies and other groups as well as attending meetings, health fairs and any other community activities
