OUTREACH SPECIALIST LEAD JOB DESCRIPTION Job Title: Outreach Specialist Lead Supervised by : Director of Pharmacy and Outreach ServicesStatus: Full-Time JOB SUMMARY:The Outreach Specialist Lead (OSL) will identify community condom distribution sites, provide HIV counseling & testing, STI screenings and referral services targeted to bring individuals with HIV/AIDS into the continuum of care. In addition, the OSL will oversee the Safe Space program and work with enrolling and engagement with patients on PrEP.DUTIES AND RESPONSIBILITIES:PrEP ServicesEnsure at least quarterly follow-up testing for clients taking PrEP and negative high risk and heard to reach clients reluctant to enroll PrEP services. Manage a caseload of clients considering or taking PrEP.Work with the Director to obtain input from representatives of the target population (CAB-Community Advisory Board) in development of plans for implementing PrEP referral.Provide PrEP/HIV related education and linkage to medical treatments and other needed services.

Provide education on Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions.Refer eligible, interested clients to primary care medical services at Priority Health Care or other health care facilities to further discuss PEP and other preventive care with a medical provider.Ensure client's charts are complete, accurate, and up to date while maintaining at least a 90% or better quarterly chart review score.Follow up on referrals to community resources in an appropriate amount of time.HIV ServicesProvide HIV pre/post counseling and testing services and STI screenings.Refer to Treatment Adherence/Partner services (SHHP) for any newly diagnosed /returning to care persons.Monitor proper storage and temperatures of all testing devices.Maintain Daily Outreach Log, Temperature Log, Control Log and Outreach Checklist Log.Conduct Early Intervention Education sessions with newly diagnosed, returning to care clients. Facilitate ARTAS (Anti-Retroviral Treatment and Access to Services and Interventions) Interventions w/ newly diagnosed/ returning to care clients.Work closely with clients by fostering adherence through education and interaction with medical teams. Assist patients with accessing services and provide warm hand off.

Consult with staff to address problems and/or concerns on behalf of patients.Identify HIV testing and condom distribution sites.Schedule patients for first on or off -site HIV evaluation with Case Manager or provider (within one week), treatment meetings, and follow up appointments.GeneralOversee Safe Space, facilitate weekly "Hot Topics" wrap sessions.Inventory and monitor testing and outreach supplies (weekly) to perform testing.Organize and administer any program incentives (e.g., bus tokens, gift cards) as directed.Attend multidisciplinary team meetings, community meetings, general staff, and other required meetings.Maintain a working relationship and involvement with area hospital and clinic social workers.Attending on and off-site training as needed.Work closely with the community to develop and maintain HIV and PrEP related networks.Participate in Outreach Activities including Pride, Southern Decadence, non-traditional venues, and other community engagement activities where the priority populations socialize and congregate.Along with the Outreach team, the OSL will refer, collaborate, assist, coordinate HIV/ STI testing and PrEP events in nontraditional settings, including local bars/clubs, and other community settings where the priority populations socialize and/or congregate.Ensure submission of required paperwork in a timely manner. Example: Testing formsComplete data entry of patient information into Evaluation Web ensuring timely submission of required reporting to the Office of Public Health, Ryan White Services & Resources, and CDC.In coordination with the Director, ensure proper and accurate inventory supply before all outreach events. Oversee the proper return and storage of materials and supplies upon return.Assist the Director with planning, implementation, and participation in community events.Report weekly database updates/issues and perform other duties as directed by the Director.OTHER RESPONSIBILITIES &ACTIVITIES:Adheres to office protocol.

(Proper clock in/clock out, work hours, voicemail, staff meetings, etc..).Interacts with clients, providers, and staff in a professional, courteous, and responsible manner.Ability to schedule work production to meet timelines with attention to detail.Must have a current driver's license, reliable transportation, and proof of car insurance to travel throughout the community as required.Maintains detailed knowledge of surroundings for both work efficiency and personal/patient safety purposes.Maintains accurate records, logs, notations, and receipts of all activities for later review and follow-up actions.Adheres to dress code; appearance is neat and clean.Complete annual education requirements.Treat clients and their families with respect and dignity.Communicate appropriately and clearly with all staff.Always maintain and ensure patient confidentiality.Report to work on time and as scheduled.Wear identification while on duty.Maintain a good rapport and cooperative working relationship with all staff.Represent organization in a positive and professional manner.Comply with all organizational policies regarding ethical business practices.
