The Outreach Nurse is responsible for providing assessment, intervention, and case management services according to MIHP protocols. Such services may be provided at the clinical site, in the client’s home, or other locations as may be deemed appropriate to client circumstances.
Requirements- Associates degree in Nursing required. Bachelor’s or Master’s degree preferred.
- Minimum one year experience providing community health pediatric and/or maternal/infant nursing services
- Current unrestricted State of Michigan RN license.
- Ability to communicate effectively with a diverse population.
- Must have strong decision making and problem solving skills.
