WeCare at Foltsbrook is actively seeking a motivated Outpatient Physical Therapistfor our community in Herkimer, NY. Our Outpatient Physical Therapist helps residents improve their mobility and move forward.

Job Description:

• Assessment of referred patients or direct access patients.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and/or families with regards to the plan of care.
• Perform evaluations, progress notes, recertifications, and discharge documentations.
• Participates in training and program development, represents the department to the medical and general community.

Shift:

Full Time

Candidate Qualifications:

• Graduation from an accredited school of Physical Therapy approved by the American Medical Association and the American Physical Therapy Association, eligibility for registration by the National Registry of the American Physical Therapy Association.
• Physical Therapist (PT) license issued by the state of New York.
• Prior experience in outpatient setting is preferred.
