Your Specific Responsibilities:

Enroll and recertify clients in AIDS Drug Assistance Program (ADAP): insurance availability & coverage; prescription(s) medication dispenses; labs; in ADAP Provide database.

Third party-payer screening. Coordinate with client care managers assisting client access with supporting resources: Medicaid, Medicare, Tricare, VA, employer sponsored insurance, ACA Marketplace, & Pharmaceutical Assistance Programs.

Client case monitoring and adherence. Referral and follow-up with dispensing pharmacies for client prescription pick up history; monitoring lab results for suppression of viral loads. Discuss adherence goals with specific to monitoring measures.

Conduct regular quality assurance activities including client file reviews and monitoring client adherence.

Remain current with changes and usage of ADAP and Ryan White policies and procedures.

Participate in ADAP, HIV treatment adherence, HIV/AIDS education/prevention and care management trainings and webinars.

Assist all internal HIV office programs with logistical functions and support.

Participate in ADAP, HIV treatment adherence, HIV/AIDS education/prevention and care management trainings and webinars.

Participate in HIV health education/prevention activities with local community.

Required Knowledge, Skills, and Abilities:

Knowledge of and experience with computers and computer software.

Knowledge of Windows (MS Word, Excel, and PowerPoint) and Outlook.

Knowledge of and experience with methods of data evaluation.

Able to review data for accuracy and completeness.

Ability to utilize problem solving techniques.

Ability to communicate effectively both verbally and in writing.

Ability to establish and maintain effective working relationships with others.

Ability to work independently and to effectively handle multiple tasks.

Ability to maintain client confidentiality.

Able to establish and maintain effective working relationships with diverse heath care clientele and constituent network.

Qualifications:

Minimum

Associates degree

Experience in interviewing, counseling, or customer services

Comfortable working with HIV+ clients.

Able to work cooperatively with healthcare professionals to meet client adherence goals.

Willing to work before, during and/or beyond normal work hours or days in the event of an emergency? This includes emergency duties including, but not limited to, response to threats involving any disaster, man-made or natural.

Preferred

Have experience working with a caseload of electronic client or patient files.

Knowledge of Microsoft Office; specifically Excel and Outlook.

In addition to English, be proficient in speaking, writing, and reading Spanish or Creole.
