Oncology Nurse Navigator
Houston Metro Urology (HMU) is the largest, single specialty urology practice in the greater Houston area. Our urologists specialize in general urology as well as sub-specialists in the areas of incontinence, female urology, robotic and laparoscopic surgery, cancer treatment of the genitourinary organs, prostatic laser surgery, and the management of kidney stone disease. HMU is well known for its advanced technology and state-of-the-art cancer treatment.
Keeping our patients abreast of the latest news in urologic care regarding diagnosis and treatment is the best way to offer, not only comprehensive care but an in-depth education and understanding of urologic health.
The Oncology Nurse Navigator is an integral part of our urology team which shares responsibilities to deliver seamless navigation and personalized guidance to patients as they move through their cancer pathway from diagnosis to survivorship.
The Oncology Nurse Navigator will oversee the identification of cancer patients within the practice population who qualify for Medicare’s Chronic Care Management services to offer care coordination and care management support.
This position will also involve some level of triage for individuals within the practice population to ensure early interventions for exacerbations of chronic conditions. The ideal candidate must be highly organized, detail-oriented, adaptable, and growth-minded.
Specific responsibilities of the Chronic Care Managerwill include:
· Working collaboratively across a multi-disciplinary team with timely communication between these entities and Primary Care Physicians to avoid fragmented care.
· Contribute to a positive experience for patients and families through courteous telephone and digital interactions.
· Facilitate referrals and linkage to community resources, financial assistance, and cancer support groups.
· Work with all clinical teams as a key resource on cancer navigation
· Contact patients to ensure compliance with treatment plans and proactively eliminate potential or actual barriers.
· Partnering with patients to establish goals, and action steps and develop individualized care plans.
· Collaborate with patients, their physician(s), and other care team members to monitor progress toward goals.
· Develop a keen understanding of urologic oncology and best practices.
· Ensure effective outreach and scheduling for recommended care and continuity of services.
· Provide patient education to increase understanding of disease processes, managing symptoms, and side effects of treatment.
· Regularly attends and participates in meetings with coworkers and practice staff.
· Collaborate with payer Case Managers for additional services when appropriate.
· Perform all job functions in compliance with applicable federal, state, local, and company policies and procedures.
· Provide advocacy and emotional support for patients, families, or caregivers from the time of diagnosis to recovery.
Background and Educational Requirements:
· Completion of a nursing program recognized by the TX State Nursing Board.
· Registered Nurse with Texas Licensure. OCN, AOCN, or CCM certification is desirable but not required.
· Minimum 2 years of direct, clinical nursing experience required. Urology or Urologic Oncology experience is a plus.
· Prior experience in care management or nurse navigation role and setting is strongly preferred.
· Bilingual preferred but not required.
Able to reliably commute to Houston for occasional in-person meetings
