Job description

The on-call scheduling coordinator is responsible for scheduling client visits with a caregiver, document communication between clients and caregivers, and ensuring office compliance with client-related documentation requirements. You will report to the on-call manager and the office manager.

Essential duties and responsibilities include the following:
Required.

· Create and maintain client service schedules as provided by the plan of care.

· Weekly call to take report prior to going On Call as well as a verbal and written hand off to the on-call manager when coming off call.

· Schedule shifts according to client preferences and Caregiver availability.

· Work with Caregivers to fill open, last minute, and emergency shifts.

· Handle requests and questions from Caregivers before, during or after shifts.

· Enter scheduling data into Company Management System/Axis Care

· Handle requests and questions from Caregivers before, during or after shifts.

· Answers phones in a timely manner using good customer service skills and judgement in the distribution of messages.

· Effectively manage scheduling changes and adjustments.

· Effectively match Caregivers and clients.

· Manage client information and updates.

· Manage Call-Offs and notify Caregivers.

· Communicate information effectively to supervisor and others in a timely manner. Call the office prior to going On Call to get report and call the office in the morning when coming off if you are still working on an open shift that needs immediate attention.

· Weekend written report documenting all pertinent calls.

· Maintain proficiency with computer systems.

· Abide by all policies and procedures set forth by the Company.

· Ensure protection of confidential information that is stored, handled, or maintained by following Company policies and procedures related to confidentiality and/or HIPAA regulations.

· Discussing service issues with consumers and assist when able and escalating all other non-emergency services calls to the on-call manager when giving report. Set expectations with the consumer; they will receive a call when the office opens from management.

· Escalate any workman’s comp concerns immediately to office manager after following the WC policy put in place by management.

· In the event of a weather emergency implement the emergency management plan based on priority call.

Schedule:

Remote- Friday at 3:30pm until Monday at 8am. The schedule may change around the holidays to ensure office
