SalaryPhysicians Choice Home Health & Hospice is looking for an experienced RN to join our team for weekday triage/On Call. Physicians Choice is a fast growing agency with competitive pay, benefits and a cohesive, teamwork driven work environment.
Job Responsibilities:
- Phone triage + visits, if needed with a schedule of Monday - Thursday 5pm - 8am or Friday - Monday 5pm - 8am; we are hiring 1 person for each schedule.
- In home hospice nursing visits as needed if phone triage is ineffective
- Monitor and report changes in patient symptoms or behavior
- Communicate with on-call providers as needed to obtain orders for patient comfort
- Maintain accurate patient medical records while following patient plan of care
- Provide compassionate emotional support to patients and their family members
- Communicate effectively to interdisciplinary team members
- Observes/ensures compliance with Medicare and State guidelines
Qualifications:
- *Previous experience in Home Health and Hospice preferred*
- Ability to build rapport with patients, families, and facilities
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
*Must be able to pass a background check and drug test.
Job Type: Full-time
Pay: $70,000.00 - $80,000.00 per year
