Position Title: Home Care Screener-Part Time

Position Summary: Participates and facilitates the discharge planning process and initial determination of home care needs for patients in all age groups who are referred for home care services. Meets with discharge planners at referring facilities to promote referrals to Oswego Health Home Care and affiliated services. Assist in day-to-day operations of the Home Care Office, patient enrollment and education to Oswego Health’s Virtual Senior Center, and assists in the facilitation of quality patient care.

Duties/Responsibilites:

  • Maintains confidentiality related to patients, family and employees, and information specific to service area.
  • Answer the telephone and route calls to proper person. When personnel are unavailable for telephone calls, records and delivers messages.
  • Interacts with others in a positive and friendly manner.
  • Responds to patient/family requests (tells patient/family what to expect, when and why; informs patient/family when problems arise).
  • Employ professional manners and tact in all dealings.
  • When callers or visitors express concern, listens sympathetically, obtains necessary information, and is courteous about interruptions caused by other calls or events and models calmness.
  • Treats consumers and co-workers with respect. Helps to diffuse feelings of anger by listening to their complaints, stating our policies and reasons for them and reporting complaints to the Manager for resolution.
  • When a patient or family is uncooperative, diffuses feelings by listening and reports these finding to Manager.
  • Enrolls prescreened clients to Oswego Health’s Virtual Senior Center. Including patient education on how to access the system.
  • Schedules and follows up with VSC clients to improve engagement and participation.
  • Upon DC from CHHA, VSC can remain active and ongoing support is to be provided as indicated.
  • Has working knowledge of phone system, fax machine, copier and computer systems
  • Answer multiple line business telephone and directs call(s) to proper person.
  • Prints necessary reports on a daily basis.
  • Verify patient benefit information and apply appropriate code for rendered medical services
  • Arrives at the job, appointments and meetings on time.
  • Selects priorities and uses work time and talents to meet them.
  • Suggests ways to improve quality and use of all resources.
  • Follows rules, policies and directives and seeks clarification if necessary.
  • Communicates with patients, family, staff and visitors in a respectful and timely manner.
  • Acts in a positive manner when change is necessary.
  • Other Duties as assigned

Education, Training, Experience, Certification and Licensure: Experience in home care with specific knowledge of the intake process. Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise.

Special Equipment, Skills, or Other Requirements: Demonstrates interest and aptitude in community home health. Demonstrates ability to make sound judgments without close supervision. Has excellent communications skills. Possesses knowledge of documentation for services rendered in acceptable format for third party payers. Must have valid driver license and reliable personal vehicle.

Work Environment and Hazards: Clinical and office setting. OSHA category I.

Physical Demands: Light work: standing, walking, sitting for prolonged periods of time, repetitive motions of eyes, hands, fingers and wrists with writing or data entry. Input data on keyboard, requires visual acuity.

Pay Range; $500/week with monthly bonus structure for generated referrals.

Employee salary is based on commensurate experience and other qualifications.
