Summary:

Occupational Therapy Assistant, able to provide comprehensive Occupational Therapy for psychiatric; chemical dependency; inpatients; outpatients; children; adolescents; adults; and geriatrics, including one-on-one treatment and group treatment. These interventions are designed to improve skills in the following areas: Activities of Daily Living (ADL), cognitive functioning, work and school performance, leisure and recreational skills, community re-entry, Independent Living Skills, sensory integration, and psychosocial skills. Perform any duties and/or responsibilities assigned by Manager or Supervisor for which qualified.

Responsibilities :

• Performs observation on patients in order to complete necessary screening, which assesses need for group services, as well the need for further Occupational Therapy evaluation and/or individual treatment services. • Formulates and documents treatment plan following collaboration with OTR/L and review of screening/evaluation data. • Modifies master treatment plan to reflect goal achievement and/or goal modification following collaboration with OTR/L. • Documents initial goals, updated goals and weekly updates on master treatment plan according to policy. • Provides individualized one on one treatment that targets goals identified on master treatment plan. • Provides group treatment that targets groups goals identified on master treatment plan. • Modifies treatment activities continuously to ensure client treatment goals are achieved. • Educates caregivers on specific strategies and recommendations that are beneficial for their children and assists with carryover implementation at home whenever necessary. • Provides patient specific information (sensory diets, recommendations, etc.) verbally or in writing to facilitate client treatment. • Formulates patient specific discharge packet based upon treatment provided and beneficial strategies noted during admission to provide to caregiver, school, etc., which will assist with carryover of treatment in all environments and facilitate functional independence of patient. • Documents SOAP notes and other progress notes according to policy. • Includes clinical data about client’s functional performance and addresses treatment goals in daily progress notes. • Completes all billing forms in full, according to policy. • Attends treatment team meeting to discuss individualized services provided, strategies identified as beneficial for patient and progress towards goals. • Attends other unit/program-based meetings in which clinical data is discussed. • Collaborates with multidisciplinary treatment team members to provide comprehensive Occupational Therapy services. • Notifies HeIS of all requests for information from the medical record. • Ensures appropriate release of information forms have been signed prior to releasing functional performance data. • Monitors safety concerns as identified and assigned by the safety office, department head, or administration. • Follows departmental; Nursing; Human Resources; and IDD’s are followed as indicated. • Completes duties and manages issues and concerns as directed by supervisor; policies and procedures; safety officer; department head; administration. • Demonstrates professional behavior toward clients; the family; the public; co-workers, supervisor; and student interns. • Develop and maintain the ability to work collaboratively with others in an interdisciplinary work environment. • Facilitates own personal and professional growth. • Attends weekly supervision to review clinical and administrative issues and concerns, as well as clients, and projects; brings an agenda. • Attends departmental and unit team meetings to coordinate comprehensive activities and clinical services. • Functions in role of Occupational Therapy consultant in order to educate other health care providers about client’s functional performance; about Occupational Therapy; and about occupational performance in general. • Productivity standard of 80% of working time is produced as billable. • Actively participates in Performance Improvement activities, in-service educational opportunities, research projects, and the student training program. • Assist in maintaining a work environment free from recognized hazards that create a risk of injury to employees, patients or visitors and that all accidents and incidents are reported by employees and properly investigated. When appropriate, assist with the return of all workers with work related injuries and illnesses to gainful employment.
