- $ weekly The Occupational Therapist is responsible for performing clientevaluations, developing and providing occupational therapy services, anddocumenting services in accordance with the plan of care developed for eachpatient.
+ Provides skilled occupational therapy services/interventions in accordance with physician orders + Reevaluates client for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriate + Assesses patient needs, plans for, evaluates and modifies care to meet goals of Occupational Therapy Interventions + Collaborates with all disciplines to plan and evaluate team goals for each patient + Instructs patient/family/caregivers on patient's total occupational therapy program + Advises, consults and instructs members of the multi-disciplinary team in specific occupational therapy modalities + Observes and reports patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team members + Refers individual to other disciplines as deemed appropriate and as approved by the physician + Teaches new skills or retrains clients in once-familiar daily activities that have become difficult or unfamiliar due to illness or injury + Establishes equipment needs to facilitate independent living + Instructs individual and/or family/caregiver in the use of various types of adaptive equipment to improve function; ensures equipment is in good working order + Trains individual and/or family/caregiver in use of prosthetic and/or orthotic devices + Performs work capacity evaluations and may refer individual to specialized vocational training in accordance with policy and regulations + Provides accurate and timely documentation according to the plan of care + Participates in care conferences or other meetings as required to ensure coordination of home health services (if applicable) + Prepares and submits clinical progress-summary based on the progress of goals + Provides in-services/education programs for various disciplines as needed + Participates in peer consultation process + Supervises Occupational Therapy Assistants according to company policies/procedures and state regulations + Participates in discharge planning when applicable + Complies with applicable local, state, and federal laws and regulations regarding professional standards and practice standards + May fabricate splints and instruct individual and/or family/caregiver in the use, application and benefits + Performs other duties as assigned/necessary Qualifications: + Current licensure as an Occupational Therapist in the State of practice + Must be a graduate of an accredited school or program of Occupational Therapy + One (1) year of prior professional Occupational Therapy experience preferred + Complies with all relevant professional standards of practice + Participation and completion of Amergis' Competency program when applicable + Current CPR if applicable + TB Questionnaire, PPD or chest x-ray if applicable + Current Health certificate (per contract or state regulation) + Must meet all federal, state and local requirements + Successful completion of new hire training as applicable to job site + Understand patient confidentiality and HIPAA requirements + Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required + Computer proficiency required
