GENERAL SUMMARY:
The Home Care Occupational Therapist is responsible for the delivery of patient care through the process of assessment, development, implementation, education and revision of treatment programs as necessary for provision of age-specific direct care/services to patients.
ESSENTIAL FUNCTIONS:
- Evaluate patient needs and set appropriate treatment plan based on measurable goals.
- Communicates effectively with patient and caregiver promoting patient satisfaction and the achievement of goals.
- Assists the patient to optimal level of physical function.
- Assists the patient to restore sensory / integrative function.
- Improves the patient's level of independence performing activities of daily living (ADL).
- Designs, fabricates orthotic and self-help devices.
- Possesses vocational and pre-vocational assessment training.
EDUCATION:
- Bachelors from an Accredited Occupational Therapist Program
EXPERIENCE REQUIRED:
- Excellent communication and organization skills required
- Physically capable of standing, walking, stooping, bending, and lifting to 50 pounds.
- Ability to problem solve, excellent written and verbal communication skills required.
CERTIFICATIONS/LICENSURES REQUIRED:
- Licensure in the State of Michigan as an Occupational Therapist is required.
- Valid Michigan driver's license and dependable vehicle insured as required by the State of Michigan.
PREFERRED QUALIFICATIONS:
- Masters degree from an Accredited Occupational Therapist Program
- Home Care experience highly preferred
