Occupational Therapist

Provides case management and direct patient care within a team environment.  Administers comprehensive skilled care in the home setting. 

• Develops and revises plan of treatments and discharge plan in conjunction with patient, family, physician, care delivery team
• Initiates appropriate preventative and rehabilitative program.
• Provides supervision of COTA to ensure patient health, safety, and compliance with plan of care.
• Consults with and educates the patient, family and other team members regarding disease process, self-care techniques and preventive strategies.  Implements home therapy programs

• Graduate of an approved Occupational Therapy program.  Possession of current OT license to practice in New York State.
• Minimum of one-year experience in a hospital inpatient setting.
• Working knowledge of the operation and minor maintenance of apparatus and equipment used in Occupational Therapy.
• Knowledge of community resources.
• Ability to function independently.

Salary Range: $70,000 - $90,000
