Obesity Medicine Physician (1099 Contractor)

FORM Health is a virtual Obesity Medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. FORM provides high-quality expert care and leverages technology to enhance the patient experience. All FORM patients work closely with their physician-led care team, which also includes advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. About the Role:
We currently have several contract opportunities for physicians who are board-certified diplomates of the American Board of Obesity Medicine. At Form, our physicians provide longitudinal Telehealth obesity care and collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. We seek applicants who:

• Have an MD degree, are board certified and diplomates of the American Board of Obesity Medicine
• Are licensed to practice and prescribe legally in California, Texas, New York, Illinois, Virginia, New Jersey, Washington, Massachusetts, Kansas and/or other states, and will prioritize candidates with multiple licenses
• Have excellent interpersonal skills for patient and team interactions
• Are committed to the highest quality of medical care and an evidence-based approach to obesity management
• Have prior experience conducting telemedicine visits
• Have prior experience working collaboratively with advanced practice professionals and registered dietitians.

Hourly Pay Range
$50 - $150 USD
