Provide primary medical care services including assessment, diagnosis, and treatment to patients of the Health Center. Provide direct patient care as the NP for the Office Based Addiction Treatment (OBAT) program if assigned.

Essential Functions/Major Responsibilities
JOB FUNCTIONS
Within the specialty of the Family Nurse Practitioner (FNP), they are responsible for managing health problems encountered by the client and is accountable for health outcomes. The FNP will follow the six standards of practice:
  • Assessment - Collect comprehensive data related to the health of the individual served. Synthesis and analysis of data and application of nursing principles and therapeutic modalities.
  • Diagnosis - Prescribing, dispensing, and administration of therapeutic devices and measures, including legend drugs and controlled substances as provided in Division 56 of the Oregon Nurse Practice Act, consistent with the definition of the family Nurse Practitioner's scope of practice.
  • Outcomes identification - Based on the diagnosis the NP will identify outcomes that are culturally relevant, health literate, based in evidence and promotes the maintenance of health. Document expected outcomes and measurable objectives.
  • Planning - Develop a plan that is client-centered and reflects best practices by identifying strategies that are achievable.
  • Implementation - The FNP oversees the implementation of the plan through treatment, case management, referrals to community partners, collaboration with care team, other health care providers and community resources, health promotion and education. Documents activities and status of care coordination.
  • Evaluation - Apply on-going assessment and analysis of data, revises plan as needed, assures that interventions and activities are appropriate for ultimate health outcomes for the client. Documents changes to enhance effectiveness of interventions
Family Nurse Practitioner provides treatment of adult, adolescents, and pediatric clients in Primary Care settings:
  • Order lab work and other diagnostic tests as indicated as part of the diagnostic evaluation or ongoing management of the client. Follows-up on all abnormal findings, tests and/or lab values.
  • Consult and coordinate with physicians, Health Center Medical Director, and Primary Care Program Manager as indicated.
  • Assure the care team is functioning efficiently.
  • Document in Electronic Health Record to systematically collect, record and analyze data.
  • Provide preventive and maintenance care to patients and families consistent with accepted primary care practice and standard guidelines.
Provide leadership, guidance, and feedback to clinic staff to support a high-performance clinic that focuses on quality and safe patient care:
  • Aid in various program areas through educating and providing consultation to staff. Helps keep staff informed of the program's overall activities and goals.
  • Serve as a consultant, accepting referrals from other health care providers in matters outside their expertise.
  • Communicate regularly with staff to review team relationships, specific problem areas and actions necessary for improvement.
  • Recognize team members' contributions and celebrates accomplishments.
Ensures quality assurance standards of care are met in programmatic areas:
  • Collaborate with Program Manager and Health Center Medical Director to:
    1. Annually review and revise health care plans, standards of care and standing orders
    2. Conduct periodic review of patient records
    3. Participate in peer reviews to assure a high level of skills among other Nurse Practitioners
  • Participate in educational meetings as available and appropriate.
  • Audit records upon request from Program Manager or Health Center Medical Director to ensure appropriate data collection and documentation and evaluates care provided by other staff members upon request.
  • Ensure quality assurance standards are met through educating, monitoring, and providing consultation to staff as appropriate.
  • Participate in the Quality Improvement Committee.
The FNP OBAT may perform additional Primary Care and OBAT responsibilities: These tasks are performed as part of the primary job functions and are not separately allocated by Frequency and % of Time.
  • Planning - Develop a plan that is client-centered and reflects best practices by identifying strategies, action plans and alternatives to meeting expected outcomes. Document the plan using health literate and culturally competent language that can be understood by care team and population being served.
  • Implementation - The FNP oversees the implementation of the plan through treatment, case management, referrals to community partners, collaboration with community resources, health promotion and education. Document activities and status of care coordination.
  • Evaluation - Apply on-going assessment data to revise plans, interventions and activities as appropriate for ultimate health outcomes for the population. Document changes to enhance effectiveness of interventions.
The FNP may perform additional Primary Care and OBAT responsibilities:
  • Facilitate access to care, gathering information for phone screening, registration, intake reporting paperwork, quarterly reports, and other required data tracking. Assist the Center to meet targeted goals and outcomes.
  • Facilitate communication between nurses, program manager, and providers, internal and external stakeholders to meet the needs of patients.
  • Provide patient support pre, during and after OBAT services; providing comprehensive Care Coordination that encompasses all relevant information on OBAT, medical, dental and behavioral health.
  • Identify and/or develop OBAT education and supportive interventions to increase patients' skills and confidence in managing their health, including regular assessment of progress and setbacks, goal setting, and relapse prevention support.
  • Educate patients and/or family members on OBAT, including medication administration, storage, safety, responsibilities, program expectations and patient treatment agreements.
  • Consistently set appropriate priorities in providing quality patient care that is organized and time efficient. Utilize staff feedback in patient care. Maintain and ensure confidentiality of patient's conditions, records, and other data on a consistent basis.
  • Oversee direct care activities of administering medications, observing and documenting patient responses and/or side effects. Administer interventions according to established standing orders and protocols.
  • Provide an environment that demonstrates caring, compassion, and respect for all patients and families. Create and maintains a high-performance clinic environment characterized by positive role model, leadership, and a strong team orientation.
  • Meet training requirements and practices according to HRSA, HHS and OHA guidelines.

Specifics: Current Nurse Practitioner Certification as a family Nurse Practitioner in the State of Oregon at the time of appointment. Possession of a National Certificate for Medicare, or the ability to obtain one by the end of trial service (6 full months from date of appointment). Employee is responsible to maintain current and active Nurse Practitioner licensure/certification and to provide verification at time of renewal.

Desirable Qualifications: One or more years' experience as a Nurse Practitioner.

  • Analyze health problems through physical exams, lab tests, counseling, and medical histories, and to recommend health maintenance or treatment plans.
  • Train clients and general public on various preventive care and health maintenance techniques.
  • Convey information, ideas, and facts both orally and in writing to supervisors, colleagues, and individuals, inside and outside the county, using language and a format the audience will best understand.
  • Utilize computers and other technology to communicate in written and verbal forms (e.g., word processing, texts, emails, spreadsheets, department-specific software).
  • Effectively relate well with others, including supervisors, colleagues, and individuals inside and outside the county. Exhibit a professional manner in dealing with others and working to maintain constructive working relationships.
  • Take personal responsibility for the quality and timeliness of work. Show up to work on time, and follows instructions, policies, and procedures. Meet productivity standards, deadlines, and work schedules.

Job Conditions and Physical Demands
WORKING CONDITIONS
Job Conditions: Normal working hours. May work evening clinics at the direction of the Program Manager and/or Health Center Medical Director. Participates in call rotation as assigned by the Health Center Medical Director.
Work Location: Lincoln County
Travel: Some travel may be required between clinic sites.

The County's current contribution rate is 11%.
Regular and Trial Service employees working between 20 and 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
