Job description

Job Description

OakWell provides high-quality primary care to patients living with end-stage kidney disease (ESKD). To ensure these patients receive quality primary care services, we provide comprehensive and integrated care right in the dialysis center. This allows patients to conveniently fit primary care into their existing healthcare schedule, keeping them healthy and living well. With an ambitious growth trajectory, OakWell is seeking and cultivating team members who are passionate about our goal of fully integrating primary and kidney care.

OakWell is powered by Oak Street Health, an innovative company delivering higher quality health and wellness care for adults on Medicare, in partnership with Interwell Health, a kidney care management company that partners with physicians on its mission to reimagine healthcare.

Role Description:

The Medical Director will join OakWell at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing, and OakWell is leading the transition from fee-for-service to value-based integrated primary and kidney care.

The Medical Director will be responsible for leading our clinicians across multiple states. The Medical Director will practice primary care alongside their colleagues on a part-time basis to enable context for the practice and to remain close to our patients. On top of a limited clinical role, they will have mission-critical leadership responsibilities, which include setting an example for how thoughtful primary care is practiced, serving as a formal mentor, coach, and advisor to providers, and participating in critical organizational-wide strategic initiatives.

Core Responsibilities:

In addition to part-time clinical duties, a Medical Director has substantial administrative time for the following leadership and administrative duties:

  • Provider Management: We want provider leadership as close to the bedside as possible. This means our medical directors are in clinics to provide management support and structure to our providers and teams. This includes ensuring clinic and on-call schedules are adequately staffed, coaching/interviewing/promoting Medical Director candidates to take local leadership roles, owning performance reviews for Providers, and supporting new provider orientations among a multitude of other functions.
  • Leadership & Coaching: At OakWell success isn’t completing the visit or hitting an R.V.U. target. Success is high quality care: (1) minimal hospital admissions, (2) excellent preventive/chronic disease quality metrics, and (3) a great patient experience. In other words: help our patients be happy, healthy, and out of the hospital. Our Medical Directors understand these ends and the means to achieve them, and they recognize these skills don’t come naturally to many clinicians; our Medical Directors share their wisdom with others and help other clinicians understand the value in value-based care. They help Providers master the data-driven tools and practices required to keep patients well and out of the hospital. Tactically this includes delivering monthly quality dashboards to teams, coaching on tools to improve quality outcomes, and ensuring Providers are engaged and responsible for the quality of care they deliver. Our Medical Directors also coach our providers on providing primary care in the context of a busy dialysis clinic, in which care teams must be flexible and supportive of their Fresenius colleagues (mostly RNs and patient care technicians) who are providing dialysis care while OakWell care teams provide primary care.
  • Support Organizational Strategy: We expect our Medical Directors to support our goal of continuous improvement. This is leadership close to the bedside, and it gives our medical directors a chance to be ears-on-the-ground as well as eyes-in-the-sky, sharing best practices and improvement opportunities with others on the OakWell team. The Medical Director’s duties are primarily focused on the smart management of their regional neighborhood, but they will have strategic input into a variety of organizational-wide projects and other initiatives which will fall into the following categories:
  • Care Management
  • Quality Improvement
  • Utilization Management
  • Clinical Committee Functions/Leadership
  • Network Management
  • Health Plan Management
  • Documentation/Risk Adjustment
  • Clinical Compliance & Policy Development

What are we looking for?

  • Physician or nurse practitioner with experience in primary care: internal medicine, geriatrics, or family medicine, especially with patients who have multiple chronic health conditions
  • A desire to provide comprehensive, holistic care to a small panel of complex patients in which quality, not RVUs, is the focus
  • Interest in participating in the development of a new innovative, quality focused care model
  • Openness to working in a dialysis center
  • Experience with risk adjustments and/or home assessments (Preferred)
  • Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-9) Coding, and Health Risk Assessments (HRAs) (Preferred)
  • Active, non-probationary state license
  • US work authorization
  • Someone who embodies our OakWell values

Why OakWell?

OakWell offers our coworkers the opportunity to be at the forefront of a revolution in healthcare, as well as:

  • Collaborative and energetic culture
  • Fast-paced and innovative environment
  • Competitive benefits including paid vacation and sick time, generous 401K match with immediate vesting, and health benefits

OakWell and Oak Street Health are equal opportunity employers. We embrace diversity and encourage all interested readers to apply.
