Your Specific Responsibilities:

This position is Tuesday-Friday 7:00 am – 5:30 pm.

The incumbent, an employee of the Florida Department of Health in Clay County, represents the agency and its mission and values. The incumbent is part of the global public health organization and is expected to promote cooperation, courtesy, and teamwork, in a diverse environment. This position is responsible for implementing the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) at several clinic sites throughout the county as directed by the Public Health Nutrition Program Director. This position is responsible for the secure collection and/ or access to records containing social security numbers.

Duties include:

Assess anthropometric, medical, and dietary data to determine client’s eligibility for the WIC program in accordance with policies of the State WIC program and regulations stated in DHM-150-24. Provides nutrition counseling to WIC participants and promotes client centered nutrition goal setting. Document services in FL WISE using SOAP charting method. Develop, monitor, evaluate and revise nutrition care plans for individual clients at high nutritional risk. Monitor and follow protocols for nutrition assessment and follow up for high-risk WIC participants. Refers nutrition counseling (first or second contact) for medically high-risk clients to licensed nutritionist. Discusses client’s progress with other professionals. Assess medical documentation forms to determine need for special formula or non-contract formula following the guidelines stated in DHM-150-24. Provide referral to other health care and social service programs. Coordinate services with other CHD programs, including immunizations, primary care, Healthy Start, etc.

Complete the Milk report each quarter. Follow guidelines for reports as stated on the Florida WIC Bureau. Identify errors and alert supervisor of corrections needed and provide supervisor with a summary of the Milk report findings by required deadlines. Assists with updating the nutrition program plan.

Complete all required forms and reports such as Employee Activity Reports, Missing Bloodwork, High Risk Contacts, attendance and leave records, travel expenses, etc. by the required deadlines.

Participate in breastfeeding promotion (via one-on-one education or outreach), breastfeeding training and activities (including World Breastfeeding Week), and participate in WIC outreach events, as needed.

Participate in the preparation and selection of information materials and presentations, including secondary nutrition education resources, bulletin boards, handouts, employee wellness, etc. for use in the WIC nutrition program and for various community groups. Participate periodically in WIC and other health surveys as required.

Aid with training of Dietetic Interns as specified by Public Health Nutrition Program Director

Perform anthropometric and hemoglobin measurements for clients during WIC certification, recertification, mid-certification, and follow ups when needed.

Provide technical assistance on program policies and current nutrition information to CHD staff. Attends required CHD meetings.

Perform other related duties as assigned and within the realm of the Classification Specifications of a Nutrition Educator.

Qualifications:

Minimum –

•           Must be able to travel within the Clay County.

•           Must have a bachelor's degree or master's degree from an accredited college or university in Nutrition Sciences, Community Nutrition, Clinical Nutrition, Dietetics, or Public Health Nutrition

•           Must have valid state driver’s license.

•           Must be able to lift and carry a minimum of 25lbs over uneven ground unassisted.

Preferred –

•           Experience working for WIC/Public Health/Community Nutrition.

•           At least 1 year work experience with Florida WIC.

•           Knowledge in the principles of breastfeeding and/or skills in breastfeeding counseling.
