GENERAL SUMMARY:

Reports to the Nursing Leader/Administration. The Nurse Coordinator is a  Registered Nurse who under the direction of department leadership develops, implements, monitors and evaluates a program of care for patients. Coordinates care across the continuum to facilitate quality outcomes. Collaborates with other healthcare team members including physicians, nurses, extenders, ancillary and support staff to ensure optimum patient care and satisfaction. Provides education to patients and family regarding disease process. Trains and directs a nursing team in the optimal care of the patient. 

Manage complex care anywhere from 300-350 advanced heart failure patients by educating patients and their caregivers of risks, complications, benefits, life-style changes as they learn to live with the diagnosis of heart failure.

Participate in weekly multidisciplinary team meetings regarding potential heart transplant and LVAD patients.

Coordinates long-term management and follow-up care in the outpatient setting for those with heart failure which includes, but not limited to lab monitoring, medication refills, testing/ procedures, BP monitoring and monitoring of weight changes.

Coordinates care and monitoring of home inotrope administration.

Monitors/facilitates implantation of CardioMEMS including device monitoring and billing.

Collaborates with Heart Failure Cardiologists and Advanced Practitioners to implement clinical interventions for all heart failure patients throughout their care continuum.


Clinical Practice: 
1. Work collaboratively with Henry Ford physicians and extenders in the care of:
2. Patients with advanced heart failure (HFpEF or HFrEF). 
3. Patients with- or suspected to have- Cardiac Sarcoidosis 
4. Patients with- or suspected to have- Cardiac Amyloidosis 
5. Patients with- or suspected to have- Heritable Cardiomyopathies - Assists with monitoring data obtained from implantable pulmonary artery
 sensors (e.g. Cardiomems)0. Regularly review data trends over time

-. Engage protocol driven medication changes and laboratory testing 
- Assists with genetic testing (filling out forms, ensuring specimens are sent for testing, obtaining results for physician review) 
- Addresses prior authorizations for medications and testing. 
- Schedules patients for testing, specialty consultation, and/or follow-up advanced heart failure clinic. 
- Answers patient phone calls, triages patient questions, and alerts providers to changes in patient status 
- Follows medication, testing and management protocols for patients with sarcoidosis, amyloid and/or advanced heart failure 
- Initiates social services, home care, physical/occupational, or pharmacy referrals. 
- Works with other members of the advanced heart failure program to ensure best care plans are devised and enacted. 

 

EDUCATION/EXPERIENCE REQUIRED: 

  • Graduate from an accredited school of nursing. 
    Successful completion of HFHS (Parent Operating Group) Pharmacology test. 
    Maintains and demonstrates current clinical knowledge in area of specialty. 
    Bachelor's Degree in Nursing preferred. 
    A minimum of three to five years of progressively more responsible clinical nursing practice in specialty. 
    Certification in specialty preferred. 
    Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans. 
    Ability to perform a wide variety of professional nursing services. 
    Ability to effectively communicate with patients, families, and medical group personnel. 
    Preferred experience in the specialty. 
    Basic computer knowledge and keyboarding skills. 
    Knowledge of Word and Excel. 

  • CERTIFICATIONS/LICENSURES REQUIRED: 
    Current licensure by the Michigan State Board of Nursing as a Registered Nurse. 
    Current BLS or ACLS per HFHS Policy.
