Position: Nurse - Home Care Services

Location: Mequon, Wisconsin

Key Responsibilities:

· Conduct comprehensive assessments and develop personalized care plans for clients and families.

· Oversee and manage all aspects of client care, ensuring high-quality and compassionate service.

· Collaborate with medical professionals and clients during hospital and rehab discharge conferences.

· Schedule, transport, and attend medical appointments with clients.

· Provide skilled nursing services and conduct home safety assessments.

· Train caregiving staff to uphold our standards of care and excellence.

· Engage in caregiving as needed and ensure caregiver performance through reviews.

Qualifications:

· Graduate of an accredited college or university with current Wisconsin nursing licensure.

· Exceptional people skills and a passion for outstanding customer service.

· Effective written and verbal communication skills, strong problem-solving abilities, and sound judgment.

· Ability to work independently, reliably, and on time.

· Enthusiasm for representing Home Helpers and engaging with the community.

· Comfortable meeting new people and maintaining valuable contacts.

Benefits:

· Competitive compensation: $38.00 to $44.00 per hour based on experience.

· Fulltime staff can earn 80 hours of paid vacation time per year.

Required Documentation:

· Nursing License

· Valid Wisconsin Driver's License and Minimum Wisconsin state auto insurance (copy of insurance card).

· Successful completion of criminal background check and Dept of Motor Vehicle background check.

· Verifiable employment history.

Territory:

· Milwaukee County (Shorewood and north, plus Wauwatosa area).

· Ozaukee County (I-43 corridor Mequon up to Port Washington).

· Washington County (Germantown and West Bend).

· Waukesha County (Brookfield, Elm Grove, and Menomonee Falls).
