DescriptionPOSITION SUMMARY

Responsible for supporting Living Innovations' mission and values within the context of policies and standards established for and by Living Innovations. Responsible for providing excellent customer service to internal and external stakeholders, employees and individuals supported. Responsible for providing employees and contracted home providers with medication training; utilizing approved curricula as well as providing clinical consultation and support to employees, individuals and home providers as needed including managing on-call urgent and emergent situations.

ESSENTIAL FUNCTIONS

Communication

  • Ensures that Living Innovations' communications are accurately and effectively conveyed.
  • Maintains open communication and cooperative work efforts with staff, supported individual's family members, guardians and other stakeholders.
  • Advocates for the needs of the individuals supported and communicates with physicians and other medical personnel on behalf of individuals.

Administrative Support and Training

  • Attends and participates in staff and team meetings as required.
  • Ensures all staff and contractors are properly oriented and trained according to state requirements, Living innovations Medication Policies and the medical needs of individuals.
  • Follows required curricula and testing requirements for mandated medication trainings.
  • Identifies general and individual specific training needs for staff and suggests these to the supervisor.
  • Provides specialized training on such topics such as diabetes, seizure management, and other specific clinical matters, as requested by the supervisor.

Compliance

  • Ensures compliance and regulatory requirements are met in an accurate and timely manner.
  • Follows standards of the Nurse Practice Act and maintains confidentiality of and individuals supported.
  • Participates in all compliance activities to maintain individual licensure:
    • In New Hampshire this includes the Developmental Disabilities Nurses of New Hampshire meetings and maintains status as a Nurse Trainer.
  • Manages on-call emergent and urgent situations.
  • Provides other consulting or clinical services, as requested by the supervisor.

Quality Assurance and Documentation

  • Documents information clearly and concisely and follows plan of care as required including but not limited to completion of Self Medication Assessments, clinical consultations, and development and tracking of PRN protocols, as needed.
  • Performs on site quality assurance checks of residential and day programs in conjunction with certification and licensing processes as required.
  • Accurate completion and timely submission of administrative tasks, including but not limited to:
    • In New Hampshire: 1201 reporting; Training documentation; QA sheets; Med error reports; HRST monitoring and timely updates.
    • In Maine: Training and testing documentation, rosters for certification process.

Qualifications
REQUIRED EDUCATION, EXPERIENCE, LICENSE, CERTIFICATION
  • High school diploma or equivalency.
  • Registered Nursing License in the state where services are provided.
  • Three years of registered nursing experience in social services or healthcare setting.
  • Valid state issued driver's license and proof of current vehicle insurance.

  • Ability to make wellreasoned decisions, both independently and as part of a team.
  • Ability to use discretion in handling confidential information.
  • Ability to use effective communication skills with all stakeholders.
  • Ability to manage, prioritize, and carry out multiple tasks.
  • Ability to work collaboratively and effectively with employees of all levels in the organization.
  • Coping skills, such as flexibility in dealing with issues typical to a social service setting.
  • Ability to meet job performance measures with minimal direct supervision and guidance.
  • Knowledge of regulatory and legislative standards related to services provided by the agency.
  • Must be able to successfully complete state provided training to become a certified Nurse Trainer in accordance with regulations.
