Job Summary:
You would be joining the Ayer Neuroscience Institute at Hartford Hospital in the position of Stroke Program Manager. This role has the benefits of being part of the Institute along with the resources and teamwork that it brings. Your role, in partnership with the interdisciplinary team, would be to support, manage and coordinate systems related to acute stroke care delivery at Hartford Hospital, optimizing the care and access. You would also be responsible for ensuring the program is operating with in Joint Commission standards for a Comprehensive Stroke Center and lead the Joint Commission Survey efforts. Supporting clinical services, tracking quality metrics and developing/implementing quality improvement plans is also a part of this role.
Job Responsibilities:
• Responds to and supports the provision of excellent stroke care. Monitors data related to quality-of-care issues and pathways for stroke care management including patient education. Reviews the process of care metrics, disseminates analyses and data summaries. Establishes a system to monitor patient satisfaction.
• Leads the Stroke Certification Process. Maintains the structure for Joint Commission Comprehensive Stroke Center recertification. Reviews data abstraction related to stroke metrics and compliance with Joint Commission and AHA standards of care.
• Develops evidence-based policies/guidelines and procedures and maintains stroke order sets and guidelines for care to enhance stroke-related medical and nursing patient documentation in the electronic health record.
• Fosters and Maintains relationships within the community stroke systems. Develops and maintains community education programs. Prepares education for outreach events, stroke screenings and lectures for local or regional events. Works with AHA and CT Stroke groups to promote stroke awareness within the local community.
• Serves as a Role Model for interdisciplinary care team. Develops and maintains staff education programs. Oversees clinical staff competencies related to stroke care.
• Develops Patient Education materials. Actively participates in stroke conferences and nursing education for stroke care.
• Maintains knowledge and stays continuously updated on new evidence and evolving practice paradigms to promote an understanding of gaps in care, resources and services. Is seen as the expert in Stroke for the Region.
• Collaboratively designs and implements quality improvement programs with defined targets and monitors results .
• Supervises and Mentors Stroke Team (Navigators and Coordinators) by supporting the team via 1:1 meetings, team meetings and daily huddle. Understands the dynamics of organizational behavior, change theory, and healthy work environments.
• Partners with Stroke Teams across the system to standardize and optimize stroke policies, procedures, guidelines, order sets and overall care for the Ayer Neuroscience Institute.
Qualifications:
• Current Connecticut Registered Nurse license required.
• Bachelor's degree required. * Master's degree preferred. One degree must be in Nursing.
• NIHSS (immediate) and SCRN Certification (or within 6 months of hire)
• Minimum 5 years of experience working with Neuroscience patients or patients with Stroke.
• CPR certification preferred.
• Demonstrated progressive leadership skills.
• Strong written and verbal communications skills required.
*Exceptions to the educational requirement will be evaluated on an individual basis. For example, a strong candidate in the process of completing the educational requirement will be considered. Continued progress toward completion of the educational requirement will be required.
