Responsibilities
The Rehabilitation Admissions Coordinator performs activities and duties related to pre-admission evaluation and admission of patients to the inpatient rehabilitation unit. The goal of the coordinator in this role is to identify patients who meet program criteria and who will benefit from an treatment in an acute rehabilitation program. Included in this role is the ability to review medical records to gather information to complete the pre-admission screening tool to review the appropriateness of the patient for admission with MD collaboration and case management to facilitate transfer in a timely manner; ensures that benefit authorization are completed as required; determines with nursing/others bed and team assignments; builds relationships with external referral sources; gathers/tracks data to be utilized in planning; ensures compliance with regulatory/accreditation organization.

Qualifications
Minimum:
ADN
AZ RN license.
Two years RN experience is required.
Must possess an in-depth knowledge of rehabilitation services regulatory/accreditation requirements.
Preferred:
Bachelors Degree in Nursing.
Previous experience in an Admissions Coordinator or Rehab Coordinator role is preferred.
Pay Range
$36.96 - $53.60 /hour
