Job description

Job Description

UltaCare Medical is seeking a Nurse Practitioner or Physician Assistant for a primary care position at a skilled nursing and long-term care senior living facility in Olney, IL. In this role, you will be the primary onsite advanced level provider responsible for the comprehensive care of the resident population. This includes addressing acute issues with urgency but also taking a pro-active approach in treating the full spectrum of chronic conditions.
The ideal candidate for this position is a compassionate and proficient healthcare professional who prioritizes patient care above all. You'll independently assess residents' medical needs and manage your schedule to ensure timely care. While autonomy is key, you'll also collaborate with the off-site Medical Director and other specialized providers, balancing independent decision-making with integrating with an interdisciplinary team.
This position offers a rewarding experience for providers who wish to utilize their broad range of skills and expertise in treating a diverse array of conditions. It is especially suited for those who desire a varied clinical practice rather than a narrow focus. Your work will have a meaningful impact on patients' lives, offering a sense of fulfillment at the end of each day.
UltaCare Medical is deeply integrated into rural communities, where our presence plays a crucial role in improving healthcare accessibility for residents who might otherwise face challenges in obtaining medical services. Established in 2006, our organization values maintaining a small company atmosphere while striving to help as many people as possible. This role is a unique opportunity to contribute significantly to the well-being of a tight-knit community.

Key Responsibilities:
  • Conduct medical evaluations, treatments, and consultations for facility residents.
  • Proactively manage chronic conditions to minimize disease progression, lessen the risk of major acute events,  alleviate side effects, and enhance quality of life for residents.
  • Address and manage acute medical situations as they arise with urgency and compassion.
  • Collaborate effectively with facility staff and the Medical Director.
  • Issue medical orders for prescriptions, medication administration, tests, and referrals.
  • Communicate with residents' families when necessary.
  • Promptly document patient interactions within 24 hours using dictation.
  • Review test results and ensure patient notification.
  • Deliver patient education as required.

Qualifications and Experience:
  • Must hold an active Advanced Level Practitioner license, open to both Nurse Practitioners (NP) and Physician Assistants (PA).
  • Capability to independently identify patients for care, set personal schedules, and establish effective relationships with facility staff.
  • Proficiency in working autonomously and making independent decisions.
  • Experience in an institutional inpatient setting is preferred.
  • Prior work in skilled nursing and/or long-term care facilities is advantageous.
  • Experience with geriatric patients is desirable.
  • Experience in other settings including traditional outpatient clinics, urgent care clinics, and other specialty clinics will also be considered.

Job Types: Full-time, Part-Time, Employee or Independent Contractor.
Salary: $105,000-$110,000 with uncapped monthly productivity bonus. (average range is additional $500-$3000 per month)
Benefits of Joining Our Team:
  • Autonomous work setting, with the freedom to make independent decisions within the facility.
  • Flexible scheduling within standard workweek hours, with agreed-upon total weekly hours for consistent facility expectations.
  • Significant impact on patient well-being.
  • Comprehensive health insurance, including dental and vision, 100% covered for employees.
  • Paid time off and sick leave provisions.
  • 401K plan with company matching contributions.

Our team of compassionate and expert providers offer a range of services, including primary care, mental health care, wound care, and more. We're committed to removing unnecessary administrative burdens and empowering our providers to deliver the best care possible.UltaCare Medical has a rich history rooted in the dedication and passion of our founder, a geriatrician, who was approached by a struggling nursing home in East TN in 2006. The facility had incurred significant tags and fines from the state for a number of issues and needed help to improve healthcare for their residents. Our founder conducted an in-depth analysis and recommended embedding certain providers into the facility to improve care. The nursing home was so impressed with his work that they asked him to implement the changes he recommended. After several years, the nursing home went from being a struggling 1-star facility, barely able to stay open, to a thriving 5-star facility at 100% occupancy. Our founder's work was recognized when he was awarded the prestigious AMDA National Medical Director of the Year. This experience inspired the development of UltaCare Medical's patient-focused model, which has evolved and improved over the intervening 17 years. Today, our team of compassionate and expert providers offer a range of services, including primary care, mental health care, wound care, and more, with a mission to elevate the standard of care for seniors in long-term care and skilled nursing facilities.
