• 2 years as a Nurse Practitioner.
• Texas state license required. Additional states is a huge plus!
• Board certified, geriatric or adult medicine specialty is a huge plus.
• Experience with HCC model, Quality HEDIS, STAR measures
• Compassionate communicator, especially with elderly and chronically ill.
• Tech-savvy with computers, tablets, mobile gadgets.

Day-to-Day:

• Dive into comprehensive assessments, all from a remote setting.
• Spot and refer routine, urgent, emergent cases.
• Suggest follow-ups with primary care doc.
• Note: Prescribing isn't in the playbook here.
• HIPAA is your second language, privacy is the anthem.
• Warm, caring vibes with members that's the spirit.
• Satisfaction of members, your scorecard.
• Safety isn't just a sticker, it's a practice.

What You Get:

• Role: Independent Contractor.
