Performs physical examinations and preventive health measures within prescribed guidelines and instruction.
Working knowledge of HIV and HEP C, and STI transmission and treatment.
Occasionally function in a mobile clinic setting including diverse locations and possibly including non-traditional clinic hours.
Works closely with the practice manager and case manager in scheduling appointments for telehealth.
Performs patient evaluation, diagnosis, initiation of PrEP for HIV prevention, and treatment of STIs utilizing the Electronic Medical Record (EMR).
Works in collaboration with a physician in formulating treatment plans for health problems, including referrals and/or follow-up.
Develops, implements, and evaluates a plan of care that is appropriate for the patient in collaboration with patient case manager.
Assisting physicians with the performing history and physical examinations including, but not limited to, dermatologic, head, ear, nose throat assessments for infectious disease.
Completes patient follow up visits to include detailed, timely documentation including patients' progress with medication and/or treatment.
Formulating treatment plans for health problems, including referrals and/or follow-up. Writes prescriptions for medication based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols.
Refers or seeks consultation with physician when a patient has a condition for which the Nurse Practitioner is not credentialed.
Assist with conducting routine and ad hoc QI/QA assessment and Peer Review.
Works in collaboration with the management team to address clinical operational issues consistency across IMG practices.
Participates in provider meetings and staff meetings as required.
Performs other related duties and responsibilities as assigned.
Computer Equipment & Software Skill Requirements:
Full knowledge of Windows Office Suite; particularly Microsoft Word and Excel
Proficiency in MS Office and CRM software (e.g., Salesforce)

Minimum Requirements:
Masters Prepared and Certified Nurse Practitioner – Registered and Licensure by the State of Florida Board of Registered Nurses
2+ years of professional work experience as a Nurse Practitioner
2+ years of experience in a primary care setting
1+ years of Disease Intervention or related field
1+ years of extensive working knowledge of STI’s/HIV/HEP C
Experience in care and treatment of HIV and HEP C and STI Transmission in a primary healthcare setting
Professional and interpersonal skill to interact with clients and health care professionals
