Paragon Clinical is a premier medical practice operating in the Chicagoland area, seeking a highly motivated, compassionate and clinically excellent Nurse Practitioner to provide PT or FT Palliative Care services to residents in Skilled Nursing and Assisted Living facilities. We are seeking Nurse Practitioners who share our values and have a passion for providing palliative services for patients in long term care facilities. NP must possess strong clinical and communication skills, and thrive working collaboratively within an interdisciplinary team. One year+ of NP experience is required. Long Term Care and/or Palliative Care experience is preferred. We require a minimum commitment of one year, but strongly desire a long term commitment with loyal and dedicated employees who can partner with us. The position is Monday-Friday with no night call, weekends or holiday work. Paragon offers a highly competitive salary and benefits package.
Responsibilities:
- Examines, diagnoses and treats newly admitted patients and existing patients
- Collaborates and communicates with Primary Physician and facility nursing staff
- Performs physical examinations related to psychiatry
- Orders, interprets, and evaluates diagnostic tests as appropriate
- Prescribes medications or non-pharmacological treatments
- Collaborates and communicates with healthcare professionals, patient and family of patient
- Records physical findings in EMR/Patient Records within specific time frame
- Submits and reports all scheduling, payroll and billing in a timely manner
- Effectively communicates with colleagues and facility staff for all facility problems
Qualifications:
- Required education: Master’s Degree
- Required license or certification: Nurse Practitioner certification, IL APN license, CS and DEA
- Required:1 year of experience
- Required: psych experience
