Nurse Practitioner

Qualifications:

• Must have active New York Nurse Practitioner license with Board Certification (PMHNP, FNP or similar).

Minimum one (1) year of clinical experience diagnosing and treating patients.

Active/non-expired DEA certification (schedule 2-5) for New York preferred.

Active personal malpractice insurance or willing to obtain one upon hiring

Primary Responsibilities:

• Provide professional evaluation and diagnosis.
• Collaborate with our multidisciplinary team.
• Provide empathetic, high-quality care to patients via video tailored to patients needs.
• Work to stabilize high-risk patients to prevent unnecessary hospitalizations and ED visits.
• Educate patients to adhere with routine physician appointments, medication adherence, exercise, and dietary management.
• Conduct follow-up online visits.
• Perform medication reconciliation if necessary.
• Develop individualized treatment plans to address the client's goals and objectives

Pay: $76.06 - $82.38 per hour
