Responsible for implementation of School Based Health Center (SBHC) program (in assigned Boston Public School) as defined by the Commission in accordance with Massachusetts School Health Regulations. The NP/PA delivers primary care services to students, acts as manager for the site, and collaborates with other members of the Commission SBHC team. High School placement site to be determined. The NP/PA establishes and maintains collegial relationships with school administrators, teachers, nurses, and staff. Works to improve optimum health and educational outcomes for all students through joint efforts of home, school, clinic, and community. The expanded services will be rendered in addition to, not in lieu of, those currently provided by the school nurse.
Performs physical examinations as related to illness or health maintenance.
Compiles medical records with complete and accurate information according to BPHC/BMC standards.
Performs basic laboratory work.
Participates in the development of clinical policies and protocols.
Counsels students regarding physical and emotional health issues and makes appropriate referrals.
Provides health education to individuals and groups in the school, clinical, and community settings.
Participates in peer review and other means of evaluation.
Provides clinical instruction to health professionals in accordance to BPHC/BMC standards of practice.
Participates in school collaboration and outreach activities as appropriate to increase SBHC utilization.
Attends mandatory Massachusetts Department of Public Health meetings.
Coordinates SBHC Flu/HPV vaccination program as directed.
Performs other duties as required.
Acts as a clinical consultant to the Division's Connecting Families to School (CFS) Program and provides public health nursing services to school age children, faculty, and staff in participating schools within Boston.
Creates and implements a general need assessment of the new school populations to be served.
Work closely with and serve as liaison to the Boston Public Schools Enhanced Services team. Services to the individual schools include health assessment, screenings, health education, counseling and referrals.
Develops a need assessment and assess epidemiologically, in collaboration with the schools, the needs of the schools and their communities to determine the health priorities of each, documenting these findings.
Creates a plan to begin to address the health needs of the schools in collaboration with the schools.
Facilitates in linking schools with community resources.
Evaluates environment conditions that are a threat to health and safety and assists those at risk to seek remedies.
Identifies students with potential health and learning needs and consults with school personnel to formulate a plan.
Links students to primary care providers as needed.
Assesses the health insurance status of children and refers to appropriate agencies as needed.
Provides health education in collaboration with school personnel
Documents nursing assessments, interventions, and evaluations in an appropriate manner.
Participates in the formation of nursing policies and public health procedures.
Participates in nursing/public health/community committees, functions and inservices.
Collects data and complete reports in accordance to BPS/State guidelines.
Perform other duties as assigned.
Minimum Qualifications
Nurse Practitioner: MSN. Certification and MA license to practice in the expanded role.
Physician's Assistant: Licensure in MA.
Eligible for prescriptive writing privileges.
2 years clinical experience working with a variety of school age groups (elementary, middle and high schools students) preferred.
Public health experience or case management/social work training a plus.
Experience in working with a culturally diverse adolescent population.
Computer proficiency helpful
Fluency in a second language helpful
