AddressSSM Health is seeking a dedicated and compassionate Visiting Nurse Practitioner to join our team in St. Louis, Missouri. As a Visiting Nurse Practitioner, you will provide comprehensive palliative care services to patients in the comfort of their own homes. This role involves collaborating with physicians, nurses, and other healthcare professionals to deliver high-quality, patient-centered care.
Responsibilities:
Conduct in-home assessments and evaluations of patients' health status.
Develop personalized care plans based on patients' medical history, current condition, and individual needs.
Perform physical examinations, order diagnostic tests, and interpret results.
Prescribe medications and treatments as appropriate.
Educate patients and their families about preventive care, medication management, and lifestyle modifications.
Coordinate care with other healthcare providers and community resources to ensure continuity of care.
Document all patient encounters accurately and in a timely manner.
Participate in interdisciplinary team meetings and case conferences.
Maintain licensure and certification requirements, including continuing education.
Adhere to organizational policies, procedures, and standards of practice.
Qualifications:
Master's or Doctorate degree in Nursing from an accredited program.
Current Nurse Practitioner licensure in the state of Missouri.
Certification as a Nurse Practitioner by a nationally recognized certifying body (e.g., ANCC, AANP).
Minimum of 2 years of experience as a Nurse Practitioner, preferably in palliative care or home health.
Strong clinical assessment and diagnostic skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a collaborative team.
Proficiency in electronic health record (EHR) documentation.
Valid driver's license and reliable transportation.
