Silver Health Care is Looking for a enthusiastic Family Nurse Practitioner to work in our Value-based healthcare delivery model. Duties Responsibilities

This role will work closely with an interdisciplinary team to provide comprehensive care to patients in our community.

· Management of complex chronic co-morbidities

· Collaboration with an interdisciplinary team

· Provide clinical / medical education o clients and staff

· Communicating test results to patients and their families

· Recording patient medical histories

Job Requirements

· Completion of recognized Nurse Practitioner program – Primary Health Care preferred

· Experience with chronic disease management and geriatric care

· Ability to deal with crisis situation promptly and effectively

· Strong interpersonal, leadership and problem solving skills
