Nurse Practitioner -Hospitalist

Overview
Position is a dual role of some NP Hospitalist duties and some NP Clinic duties working in the same facility in Harvard, IL
Hospital: Works collaboratively with dedicated and caring hospitalist physician group to provide evaluation and treatment of patients in an established hospitalist program.
Clinic: Provides general medical care and treatment to patients in medical facility such as clinic, health center, or public health agency, under direction of Physician, by performing the following duties.
**Call required 3-4 nights per week, no weekends
Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES -
Hospital

• Patient admissions, completing progress notes, patient education, prescribing medications, order diagnostic tests, and recommending therapeutic interventions.
• Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.
• Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
• Records physical findings, and formulates plan and prognosis, based on patient's condition.
• Rounding on Long Term Care patients in the Care Center who are having acute health episodes with support of a physician.
• Helps to manage TCC patients with assistance of nursing staff and specialist team.
• Manages GI patients, submits orders and conducts phone call follow up with the assistance of GI physicians.
• Discusses case with Physician and other health professionals to prepare comprehensive patient care plan.
• Submits health care plan and goals of individual patients for periodic review and evaluation by Physician.
• Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
• Refers patients to Physician for consultation or to specialized health resources for treatment

CLINIC:

Education and Experience

• Graduate of an accredited nursing program. Possession of a current applicable Illinois RN license and IL Advanced Practice Nursing License.
• Meets the requirements determined by the State of IL to perform duties defined as those of a Nurse Practitioner
• Two years of Hospitalist experience preferred

Certification/Licensure
CPR and ACLS required
NP must be able to work with all ages of patients
