About Us:
We are currently seeking a motivated and experienced Nurse Practitioner to join our TCP team. As a Nurse Practitioner, you will play a vital role in ensuring smooth patient transitions, providing critical care coordination, and delivering exceptional healthcare services under the supervision of a Board Certified Internal Medicine MD.
Responsibilities:
- Conduct assessments and evaluations of patients' medical conditions during care transitions.
- Collaborate with interdisciplinary teams to develop comprehensive care plans.
- Coordinate and communicate with patients, families, and healthcare providers to ensure continuity of care.
- Provide patient education, medication management, and post-discharge follow-up.
- Utilize telehealth tools to conduct virtual visits and consultations.
- Document patient interactions accurately and timely within the electronic health record (EHR) system.
Nurse Practitioner Responsibilities:
- Performing physical exams.
- Recording patients' medical histories and symptoms.
- Creating patient care plans and contributing to existing ones.
- Ordering, administering, and analyzing diagnostic tests.
- Consulting with healthcare professionals and families.
Nurse Practitioner Requirements:
- Board-certified or eligible by either ANCC or AANP.
- APN authorization in the state where employed to practice in family health by the Board of Nurse Examiners.
- A sound understanding of all pertinent federal and state regulations, including HIPAA and OSHA.
- Excellent verbal and written communication skills.
