Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the under served communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually.
We are seeking a Family Nurse Practitioner for our Manhattan and Brooklyn sites.
Key Information:
- Job Type: 35 Hours per week (Full-time).
- Compensation: ranges between $123,758.08 To 132,649.15 / annually based on experience.
- CHN is a qualifying employer under the federal government’s Public Service Loan Forgiveness Program.
- Excellent Benefits and PTO package:
- Vacation accrual is 20 days per year.
- 4 Personal Days per year.
- 9 Holidays per year.
- One paid Sick Leave day per month (12 Sick Leave Days annually). Unused portions of Sick Leave get carried over from year-to-year and can be built up to 60 days.
Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.

WHAT WE LOOK FOR:
- Current N.Y.S. License to practice as Family Nurse Practitioner.
- At least one (1) to two (2) years of post- graduate/residency experience delivering care in a Primary Care/ Ambulatory Care setting.
