St Marys primary care network consists of 7 health centers located in Fulton and Montgomery counties. Each of these centers work with multidisciplinary teams to provide optimized, patient centered- care tailored for each of the patients we serve. We are seeking a board certified/board eligible FNP to join our primary care team. This is a hospital employed, outpatient setting
Responsibilities:
- Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening and patient care.
- Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
- Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
- Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Personal Protective Equipment: • Follows Standard Precautions using personal protective equipment as required.
Required Credential(s):
- Certified and Registered Nurse Practitioner (NP) by the New York State Education Department (NYSED), obtained prior to start date.
- BLS Provider obtained within 3 Months (90 days) of hire date.
