Your New Mission:
- Hit the road, delivering unmatched care to nursing home residents throughout Columbia, South Carolina. This is more than a job; it's a calling!
- Make impactful connections and provide essential care within local communities.
- Licensure as a Nurse Practitioner or Physician Assistant in South Carolina, ready to expand your horizons.
- National Board certification (AANP, ANCC, or NCCPA) - proof you're elite in what you do.
- DEA License and a solid understanding of Medicare.
- Your own vehicle, a valid South Carolina driver's license, and comprehensive insurance. We prioritize your safety on the road!
- Perform in-depth assessments and manage the care of our members with empathy and expertise.
- Apply our unique care model, emphasizing in-home treatment and tailored care plans.
- Work closely with other healthcare professionals to ensure seamless, high-quality care.
- Act as a trusted consultant to families and nursing facility teams, guiding them through healthcare decisions.
- You have at least 6 months of relevant experience, particularly with geriatric, hospice, or palliative care.
- Tech-savvy and exceptional communicator.
- Independent, yet thrive in team settings, with strong critical thinking and organization skills.
