Nurse Practitioner
Summary
This position includes administering select prevention, wellness and monitoring services to older adults and helping them understand the effects that health maintenance can have on their well-being now and in the future.
Scope of Practice
- The Welcome to Medicare Visit (IPPE)
- The Annual Wellness Visit (initial and subsequent)
- Functional ability, fall risk, hearing, depression, vision and cognitive impairment assessment
- Height, weight, BMI, and blood pressure
- Review of individual, family medical and social history
- Review of current medication and supplements
- List of risk factors and conditions for interventions and treatment options
- Personalized preventive recommendations
- Personalized preventive health service planning
- Health education
- Advanced Care Planning, as requested
- Review patient results
- Referral to medically necessary ancillary services
- Remote Patient Monitoring coordination, including but not limited to discussions on benefits/risk, consent, financial responsibility, care plan development, patient education and planning and management of critical results
- Ordering preventive/screening services as required
Job Responsibilities
- Hold and maintain an average of 20 state licenses
- Manage and complete state required continuing education
- Collaborate with Nurse Practitioners/physicians as needed and as required by state and contract
- Provide feedback to the Administrative Team on state license requirements
- Manage daily schedule and complete assigned patient visits and tasks
- Manage no shows and cancellations with an effort to reschedule visits
- Complete required charting consistent with medical standards
- Consistent, constructive and effective communication with team, patients and leadership
- Provide education and resources to patients as related to their specific needs and conditions
- Discuss financial responsibility with patients as relevant to the service being offered or provided
- Perform eligibility checks as needed
- Work assigned days and hours, providing a minimum of 2 months’ notice for change in schedule
- Participate in the growth and development of the business
- Minimal on call as needed and agreed upon
- Added tasks as necessitated by the business
Job Skills & Qualifications
- Active unencumbered, board certified APRN license, or equivalent
- Authority to write orders for preventive services (RX Authority may be required depending on the licensing state)
- Ability to work independently in a remote, virtual and private environment and have access to a reliable wireless network
- Strong work ethic
- Positive Attitude
- Detail-oriented
- Excellent written and verbal communication skills
- Readily able and willing to learn new things.
- Excellent computer skills and knowledge, EMR experience preferred
- Ability to work in a high volume, fast paced environment
