Job Description
We are sourcing for a Nurse Practitioner with Infectious Disease experience to join a busy infection disease practice in Fort Worth!
The Nurse Practitioner is responsible for providing primary and preventative medical care to individuals enrolled in client's clinical programs, in accordance with standards set forth by client's policies and procedures, JCAHO Nursing Standards, CRNP Scope of Practice and Standards and the Department of Health.
HOURS:
Monday-Friday 8am-5pm, and different half days.
Bonus structure full benefits after 90 days- PTO, holiday pay, 401k after 1 year
PRINCIPAL DUTIES AND RESPONSIBILITIES
EMR: ECW
Perform history and physical exam.
Determine and order appropriate laboratory and diagnostic tests when needed.
Review and interpret results of laboratory and diagnostic studies and collaborates with the Medical Director regarding identified concerns.
Determine and order pharmacologic, intravenous and other therapeutic interventions as needed.
Identify medical conditions that require further evaluation by the physician or other specialist.
Plan interventions, which are consistent with the needs and patients’ goals and protocols.
Identify resources and coordinates referrals for adult patients requiring further evaluation or services.
Maintain current medical records for all patients and provides documentation of clinic visits.
Collaborate with other team members in formulating an appropriate plan of care for each patient.
Provide ongoing assessment of medical, psychosocial, and educational needs. Evaluate the effectiveness of the patient care plans and modify interventions accordingly.
Provide telephone triage for patient related concerns/issues.
Communicate with the referring provider or specialists.
Determine patient eligibility for clinical trials and other research protocols and refers patients accordingly.
Performs other related duties or special projects as assigned
QUALIFICATIONS:
Master’s degree from an accredited nursing program; Licensure to practice as a certified registered Nurse Practitioner in the state of Texas. At least two years’ experience in an advanced practice nursing role. Experience in HIV/AIDS preferred. Must be eligible for Medicaid/Medicare provider status
KNOWLEDGE, SKILLS, & ABILITIES:
Demonstrated experience providing primary care as a family Nurse Practitioner; Must have prescriptive authorization
Experience working with a high-performance, collaborative, constructive agency.
Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community.
Ability to collaborate with members of culturally and professionally diverse staff.
Sound judgment when dealing with patients, staff and community at large.
Culturally sensitive to medical problems and social needs of underserved populations
Sound understanding of all federal and state regulations including HIPAA and OSHA.
Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
