The Boston Medical Center Section of General Internal Medicine is seeking an NP to provide primary care while also working with the WellSense Health Plan Senior Care Options Care Management team.
This is an exciting time to join the Adult Primary Care Practice at Boston Medical Center as a Nurse Practitioner (NP) and be a key member of a team-based, NCQA-certified patient centered medical home. We are hiring NPs with experience in outpatient primary care who will provide evaluation and management of patients with acute and chronic illness in our academic Internal Medicine practice. This specific NP position is unique due to its combination of direct patient care and partnership with the health plan.
Key Functions/Responsibilities:
- Conducting assessments including detailed history, performing physical examination, and documenting appropriate diagnosis to ensure capture of high-risk codes under CMS guidelines and Rating Category under MassHealth guidelines
- Perform urgent medical visits to ensure that members are given timely and appropriate medical care in order to avoid emergency room or hospitalization
- Perform in clinic post-discharge visits on members within 24-48 hours of discharge from an acute care facility or skilled nursing facility
- Maintain expert knowledge of the MDS-HC data
- Review MDS for training and quality assurance purposes and collaborate with the WellSense SCO Clinical managers
- Participate in workgroups that promote clinical excellence, and participate in activities and education to maintain and advance competency
- Serve as a lead consultant on complex care planning during SCO rounds
- Coordinate with Nurse Care Managers at WellSense SCO regarding DME requests, and review and provide appropriate documentation
- Formulate and monitor a treatment plan based on scientific rationale, evidence based standards of care and practice guidelines that demonstrate critical thinking, diagnostic reasoning and knowledge of pathophysiology of acute and chronic diseases
Requirements:
We are seeking an NP with 3-5 years of outpatient experience, preferably with the geriatric population and/or Psych, and an NP with past experience, or interest in establishing a bi-directional working relationship with managed care payers. This position is ideal for those who thrive in a fast-paced environment and love direct and indirect patient care.
