Job description

The Hospice Nurse Practitioner works in collaboration with the medical director, and other hospice care team members, to assess and evaluate patients for ongoing hospice services and offer strategies to improve care for patients and families. This position reports to the Director of Hospice. The NP conducts the Face to Face visits with hospice pts per CMS requirements. The nursing process will be utilized (assessment, planning, intervention, evaluation, and teaching) in providing care. The NP will demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrate knowledge of the principles of growth and development and possess the ability to assess data reflective of the patient's status. Interpretation of information is required to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed as described in the department's policies and procedures. The Nurse Practitioner performs beside hospice care patient/family evaluation; assists with diagnosis, treatment and management of health conditions impacting hospice care; Consults ongoing with medical director as needed to ensure high quality patient care. Provides training and continuing education for staff and assists in development of clinical practice guidelines/standards in support of quality hospice care. Performs other job duties as assigned. Complies with applicable laws and regulations with respect to the scope of advanced practice nursing and hospice regulatory guidelines.


Performance Expectations:

  • Demonstrate ability to assist nurses with caring for patients using wound VAC and troubleshooting equipment as needed.
  • Demonstrate ability to oversee medication administration in the home setting by oral, peripheral and IV pump administration.
  • Demonstrate ability to enter all patients charting in EMR according to policy.

Qualifications:


Education/Skills

Graduate of a recognized non-online RN program required. Successful completion of a Master degree in Nursing.


Work Experience:

Minimum of 5 years nursing experience required. Clinical nursing care in Oncology, hospice, geriatrics, or intensive care preferred.


Certification/Licensure-DUE UPON HIRE


  • Licensed RN able to practice within the State of MS as an advanced practice nurse.
  • Certified as a Family Nurse Practitioner by the American Nursing Credentialing Center (ANCC) required.
  • Valid Drivers License and proof of automobile insurance required.
