Responsibilities
GENERAL PURPOSE:
Provides support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
JOB DUTIES AND RESPONSIBILITIES:
Patient Care Interface (through EPIC and by phone)
- Develop relationship with patients, maintain collaborative relationships with families/caregivers, multidisciplinary provider(s), nursing leadership and staff and other internal service delivery departments. Provide ongoing emotional support and coordination of care for patient
- Assures continuity of care: communicates pertinent information regarding patient issues with other team members i.e. Procedure Suite Nurses, regarding anticoagulation communication; recommends services consistent with the patient's care needs and benefits; collaborates with schedulers to ensure all medical information is complete and available for visits including pre-visit diagnostic results and manages efforts with goal of having measurable impact on improving patient's overall quality of life.
- In basket responses, support and management (specifically, managing patients who send multiple messages escalating through MyChart)
- Service recovery for the Interventional Spine Program and Providers, including addressing transfer of care requests
- Second opinion consult review (request prior records/imaging), including a set protocol that "flags" the NN to review (h/o pain pump, spinal cord stim, prior treatment from other pain clinic, etc.)
- Address questions pre- and post- procedural for Medial Branch Blocks, Radiofrequency Ablation and other complex procedures
- For trials and OR cases contact patients one week in advance
- Triaging referrals and urgent follow ups
- ensuring procedure time is filled appropriately and interfacing with ortho scheduling group ; scrubbing schedules

QUALIFICATIONS:
Bachelor in Nursing required. Three to Five years of Nursing experience required or equivalent combination of education and experience. Required competence in critical thinking, interpersonal relationships and technical skills. Licensure as a Registered Nurse in NYS; certification in a Nursing specialty area preferred.

Pay Range
Pay Range: $75,650 - $98,342 Annually
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
