Job description

The Nurse Navigatorwill function as an OBGYN Nurse Navigator, as a member of the multidisciplinary team and serves as an advocate and educator for patients, from diagnosis through end of active treatment. The Nurse Navigator's primary function is to ensure the patient remains compliant to the treatment plan. Navigators achieve this goal by building relationships with patients and physicians, coordinating the plan of care, assisting with appointments, transportation needs, education, resource provisions and/or representation within the multidisciplinary care environment. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for patients, including support with interdisciplinary and cross facility coverage and collaboration. The Navigator will serve as a liaison between the patients and family, all physicians involved in the patient's care, internal and external healthcare providers, support network members, and the wider healthcare community. This role will require collaboration with local physician liaisons and leadership to conduct internal and external outreach and marketing.
Pay range for this position is $32.64-39.02 per hour, depending on years of experience.

MAJOR RESPONSIBILITIES
  • Nurse Navigator
    • Serve as patient advocate from diagnosis to end of active treatment
    • After notification via physician, initiate contact with patient at time of diagnosis to introduce navigation program & navigator role
    • Be assessable to patients and family members throughout the care continuum, and be responsive, knowledgeable, and empathetic regarding all care needs
    • Respond to patient challenges/barriers to care until resolution is achieved
    • Assess patients medical, social, psychosocial, and other care needs
    • On an individual basis, use appropriate tools to identify patients' needs and barriers to care and provide access to potential resolutions
    • Identify health disparities and assist in removal of these disparities
    • Provide appropriate teaching, outreach, and support to patients and families. Ensure the patient is empowered to manage his/her own healthcare needs
    • Provide clarification on the healthcare system throughout the care pathways
    • Support providers to assist patients in understanding their diagnosis, treatment options, and the resources available, including education, clinic research studies, and technological advances
    • Provide education/connection to resources on subjects that fall beyond the scope of individual modalities (access to supportive care, financial support, return to work)
    • Streamline processes for patients by assisting with appointment scheduling and paperwork preparation
    • Ensure the organization of appointments and explain the sequence of treatments
    • Ensure smooth transitions between care modalities, facilities, and providers. Introduce patients to appropriate caregivers, as needed
    • Facilitate patient movement through the appropriate clinical pathway and collaborate with physicians to ensure patient compliance
    • Coach and assist patients to remove barriers related to insurance coverage, transportation, childcare, finances, language, etc. so they can focus on their treatment plan, not these barriers
    • Connect patients to hospital and community resources
    • Conduct follow up conversations with all patients and communicate any concerns, changes, or social needs to the appropriate physician or other care provider
    • Attend Multidisciplinary (MDM) and/or other meetings as necessary
    • Ensure appropriate patient date is available and patients are appropriately assessed and documented upon, including identification of appropriate clinical research study options
    • Serve as patient advocate and multidisciplinary team member at these meetings
    • Document throughout patient care continuum in inavigate database
    • Collaborate with cancer registrars, physicians, and other team members to ensure data collection is timely and accurate
    • Drive process improvement
    • Collaborate with Director of Navigation Operations to determine successes and opportunities related to monthly navigation scorecard report
    • Make appropriate recommendations for changes to the current program, both locally and at a corporate level, and assist in delivering program improvement
    • Conduct outreach to referrers, provides and other medical professionals as well as to the corporate 'customer' community
    • Establish and maintain positive working relationships with key internal and external customers (including physicians, nurses, radiology staff, social services, radiation oncology, business office etc.)
    • Educate each constituent on the role and benefits of the navigation program and high quality cancer care
    • Recognize scope and limitations of role and regularly access Director of Navigation Operations as a support to the role
    • Provide referrers with timely feedback and patient progress
    • Stay current on the latest nursing developments and participate in organizational conferences
    • Ensure service continuity
    • Establish appropriate mechanisms to ensure service continuity during both planned and unplanned absences and undertake succession planning
    • Mandatory - Practices and adheres to the "Code of Conduct" philosophy and "Mission and Value Statement"

    • Implement strategies to sustain efforts such as audit tools and meaningful feedback
  • Fiscal Responsibility
    • Consider the financial/safety implications and clinical outcomes when making recommendations, evaluating technology and products, and developing policies and procedures
    • Use a systematic approach to evaluate costs, benefits, and efficacy
    • Incorporate fiscal assessments into program evaluations and/or reports
    • Develop and maintain department budgets

EDUCATIONAL REQUIREMENTS
  • Licensed graduate from an accredited school of nursing
  • Must maintain current BLS

QUALIFICATIONS
  • Knowledge of medical terminology and procedures
  • Knowledge of computer systems to be able to utilize the internet, EMR and email
  • Knowledge of Nevada Medicaid rationales for case management billing
  • Knowledge and development of a written plan of care
  • Knowledge of insurance billing and insurance practices
  • Knowledge of business principles involved in planning, resource allocations, human resource concepts, operations, policies and procedures
  • Skills for management of personnel resources - motivating, developing and directing people as they work
  • Strong leadership skills to promote teamwork and excellent customer service practices.
  • Strong communication skills
  • Skills to effectively deal with and negotiate through confrontational situations and stressful interactions
  • Skills for using the computer and software: Word, Excel and Outlook
  • Ability to maintain confidentiality and follow PHI/HIPAA regulations

  • Ability to perform nursing duties efficiently, accurately and safely
  • Ability to interpret and read documents such as safety rules, operating and maintenance instructions and procedural manuals

  • Must maintain current BLS
