Under the general direction of the Nurse Practitioner or the Chief Medical Officer, the Nurse Navigator serves as primary palliative care nurse for assigned patients, ensuring that each receives appropriate services. The Nurse Navigator provides direct nursing care and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers, facilitate flow through, increase satisfaction, and promote Palliative Care services within the defined service community.
Job Description
- Respond to calls from patients and family in a timely and professional manner, whose primary need is palliative in nature. Performs nursing assessments and triage for new and ongoing patients. Functions as a central point of contact for navigated patients including interaction with medical, nursing, ancillary services, and hospice if needed.
- Provide education to patients/families; act as an information resource.
- Perform nursing duties according to Palliative and Hospice Nursing standards of practice and HGI/ GIPC policies and procedure. Monitor care plan, compliance, coordination and referrals to resources in collaboration with the care community, Nurse Practitioner and CMO.
- Display critical thinking skills to problem solving and manage patient and family issues according to Palliative and Hospice Standard of Practice and policies and procedures.
- Maintain excellent direct care nursing skills.
- Complete required documentation in an informative and descriptive manner on each patient daily.
- Develop and maintain a good working rapport with all internal and external personnel to ensure that nursing services and resources can be adequately maintained to meet the needs of the patient and family.
- Complete required documentation upon admission, transfer, discharge, or referral of patient.
- Assist with marketing and public education to promote GIPC.
- Develop, implement, review and update an interdisciplinary plan of care which addresses medical and social needs.
- Monitors patient's treatments and medication.
- Initiate communication with attending physicians, other palliative and/or hospice staff members and other agencies as needed to coordinate optimal care.
- Assist other nurses/case managers. Inform the NP and CMO of unusual or problematic patient/ family issues.
- Demonstrates familiarity with policies, and State and Federal regulatory requirements.
- Assist with GIPC On Call coverage as needed.
- Maintain up to date records using the EMR system so problems, plans, actions and goals are accurately and clearly stated according to policies, best practice and regulatory requirements.
- Answer patient and families questions about medical care and the end-of-life process.
- Successfully completes the annually required in-service as assigned
- Participates in QAPI program as appropriate.
- Displays critical thinking skills to problem solve and manage patient and family issues according to Palliative and Hospice and Standard of Practice and policies and procedures.
- Maintains the confidentiality of all patients.
Education & Certifications
- Current RN license, State of Georgia.
- Current CPR certification, driver's license and auto insurance coverage.

- Knowledgeable of nursing practices and procedures.
- Ability to make independent decisions when circumstances warrant such action.
- Ability to assess and manage pain and other symptoms of discomfort in patients.
