Job Summary:
Howard County Medical Center invites applications for the position of Nurse Navigator at its location in St. Paul, Nebraska. The Nurse Navigator's primary function is to coordinate and manage patients chronic conditions to improve the overall health of the patient with supervision of the Medical Staff and Clinic Director. Additionally, will perform Medicare's Annual Wellness Exams and regular clinic nursing duties as needed.
***Mileage Reimbursement for candidates outside a 25-mile radius from the St. Paul location!
Essential Duties:
- Utilizing available technology, maintain and support multiple data bases to help meet quality measures and to help maintain health and wellness for patients.
- Primary nurse for completing Medicare's Annual Wellness Exam
- Develop and assess reports and provide information on quality metrics to interdisciplinary team.
- Assist and develop care plans for patients who have chronic conditions.
- Evaluate and educate patients on optimizing their health with Chronic conditions
- Communicate and coordinate care with an interdisciplinary care team.
- Contribute to a team to meet value-based care and improve the overall population health.
- Provide additional back up help for the medical clinic.
- Implement health education in the community.
- Performs all other duties as assigned.
Qualifications:
Education:
- Current Nebraska Registered Nurse Licensure is required
- Associated Degree of Nursing required, but bachelor's degree is preferred (or willing to obtain).
Experience:
- Three to five years of nursing experience is required, preferably in a Rural Health setting.
- Experience with computer and data collection is required.
