Nurse Liaison

Position Summary:

• Promotes LifeBridge Health Post-Acute Services to system hospitals and external facilities, patients, families, and providers.
• Completes patient assessment to determine level of care needs and recommends admission to appropriate post-acute services.
• Serves as liaison between patients' current provider and LifeBridge (LBH) post-acute services, working to convert patients for admission.
• Responsible for marketing LBH Post-Acute Services to assigned territory.

 Essential Functions:

• Evaluates patient referrals from physicians, social workers, case managers and family members for potential admission to LifeBridge post-acute facilities.
• Completes face to face assessments to determine appropriate level of care for referred patients

Tasks:

• Visits health care facilities to complete a pre-admission assessment.
• Evaluates nursing and medical needs of patients in acute care.
• Maintains a current, comprehensive, and accurate record of all patients evaluated for placement.
• Responsible for territory management and planning.
• Completes and distributes daily log in a timely manner.

Qualifications/Requirements.

• Formal working knowledge; equivalent to an Associate's degree (2 years college)
• Requires knowledge of a specialized field Education/Knowledge: Experience: 3-5 years Technical/Clinical Skills (Usage):

Certifications, Licenses, Registrations:

• Associate's degree required, Bachelor's degree preferred
• Preferred: RN, LPN, RRT, PT or other related clinical license/profession
