Essential Functions
In collaboration with the Registered Nurse:
Provide basic nursing care related to safety, nutrition, hygiene, elimination, and mobility.
Take and record vital signs, including apical pulse rates.
Change dressings, sterile and non-sterile.
Administers medication, including IV medication, using the Five Rights and documents and communicates, clinical findings to the RN and Provider.
Urinary Catherization.
Perform wound irrigation, colostomy care.
Perform a basic physical assessment and initiate the admission assessment.
Insert, irrigate, and remove NG tubes.
Administer tube feedings.
Perform blood glucose testing.
Perform other nursing skills as defined by their school of nursing competency checklist or has demonstrated competency in under the supervision of the assigned RN.
Contribute to the development of a plan of care and supporting documentation.
Assist in the admission and discharge of patients, documenting the work completed.
Assist with patient education, documenting education provided.
Document pertinent information on the patient record with RN co-signature on Nursing Notes and other Clinical Documentation.
Assist patient with activities of daily living and personal cares, documenting the activities and outcomes.
Uses the nursing process to work in collaboration with the healthcare team, implementing and documenting individualized nursing plans of care, including discharge planning and patient/family teaching, incorporating age considerations.
Maintains a safe, comfortable and therapeutic environment for patients/families according to department/facility standards.
Cleans and maintains equipment and environment.
Responsible for cost-efficient use of equipment and supplies to provide quality patient care.
Seeks learning experiences - participates in education offerings and meetings, reads current literature.
Receives and provides feedback, both positive and negative in a mature, professional, and non-judgmental manner.
Demonstrates interpersonal skills that allow the individual to relate well to all patients/families, employees, referral agencies and the public. Supports/leads teamwork within a patient centered team.
Demonstrates self-motivation and good organizational skills.
Demonstrates excellent communication skills with written and oral communication.
Practices and maintains universal precautions while performing direct patient care duties.
Responds to emergency situations, demonstrating a calm demeanor and appropriate actions.
Identifies and takes corrective action of safety situations not consistent with routine operations of department/facility.
Complies with reporting requirements of patient rights and HIPAA concerns.
Contributes to an environment in which the patient care team can work cooperatively toward desired objectives.
Keeps current with new knowledge and information through formal and independent learning activities to ensure patients will receive up-to-date care planning.
Demonstrates competence performing job duties.
Demonstrates professional appearance and attitude that support Spooner Health's Mission, Vision, Values and Standards.
Attends all mandatory meetings and 75% of monthly staff meetings.
Completes mandatory/required in-services/trainings within the specified time.
Completes other duties, as assigned.
Demonstrates basic knowledge and use of computers by documenting delegated nursing duties, including passing medications and performing treatments.
