Overview
Position Summary: The Nurse Care Manager performs complex discharge planning for patients of different ages and developmental levels with serious illnesses. focuses on patients with complexities related to finance and/or resource utilization. Acts as an advocate for the patient or family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes and customer satisfaction in a cost-effective manner. participates in or facilitates the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.
Minimum Qualifications: 6+ years of pediatric nursing experience and/or ambulatory setting experience with advanced level pediatric clinical experience, utilization management, and/or discharge planning experience are requiredExperience with insurance and authorizations is preferred. Teamwork requires flexibility, the ability to multitask, and time management. Thorough knowledge of managed care, utilization management, discharge planning, and continuous quality improvement concepts. Computer Literacy. Strong interpersonal, organizational, prioritization, negotiation, and critical thinking skills. Fluency in both English and Spanish is desired.
Education/Licensure/Certifications: BSN is required, as are licensure and BLS/PALs certification. A master's degree in nursing, health care administration, or a related field is preferred. Currently, registered nurse licensure in California.
Pay Scale Information
USD $99,424.00 - USD $171,506.40

Final determination of a successful candidate's starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
